BACKGROUND Prostate cancer is the second most common cause of cancer related deaths among men in the United States. In India, prostate cancer affects 4-5 men per 100,000 population 1. Although it is a common disease, it is slow to manifest clinical signs. It is estimated that 30% of men over 45 yrs, harbor latent prostate cancer and will remain latent until death. The objective of the study was to evaluate the role of Transrectal Ultrasound (TRUS) guided biopsy with histopathological evaluation (HPE) in the detection of prostate cancer on the basis of elevated prostate-specific antigen levels (PSA). METHODS This prospective study was done on 36 patients who were referred to department of Radio-Diagnosis at Yenepoya Medical College Hospital, Mangalore for TRUS guided biopsy. TRUS guided biopsy was performed using the sextant core biopsy technique & samples were sent to the department of pathology for histopathological evaluation and results were documented. Results were analysed based on frequency and percentage. RESULTS Prostatic diseases were more common in the older age group and incidence increases with age. Most of the patients in our study had grade II prostatic enlargement. Our study showed diagnostic rate of 6 core biopsy with serum PSA levels 4-10 ng/ml was in 11.1% cases, while the diagnostic rate was 0% with serum PSA levels 11-20 ng/ml, 16.75% levels of 21-30 ng/ml, 25% with levels of 41-50 ng/ml and diagnostic rate was 100 % with serum PSA levels of >51 ng/ml, indicating strong correlation of PSA level with tumour diagnosis by TRUS guided biopsy. CONCLUSIONS TRUS guided sextant core biopsy is a safe and effective procedure in diagnosing prostate cancer and has high diagnostic rates for patients with PSA levels of >50 ng/ml and gland volume between 30-50 cc.

Is There a Role for Body Mass Index in the Assessment of Prostate Cancer Risk on Biopsy?

Purpose To examine the role of body mass index (BMI) in assessment of prostate cancer (PCa) risk. Materials and Methods 3,258 participants who underwent biopsy (including 1,902 men with a diagnosis of PCa) were identified from the Selenium and Vitamin E Cancer Prevention Trial. The associations of BMI with PCa and high-grade PCa (HGPCa) were examined using logistic regression, adjusting for age, race, BMI-adjusted prostate-specific antigen, digital rectal exam, family history of PCa, previous biopsy history, PSA velocity and time between study entry and the last biopsy. The prediction models were compared with our previously-developed BMI-adjusted Prostate Cancer Prevention Trial prostate cancer Risk Calculator (bmiPCPTRC). Results Of the study subjects, 49.1% were overweight and 29.3% were obese. After adjustment, among men without a known family history of PCa, increased BMI was not associated with higher risk of PCa (per one-unit increase in logBMI: OR=0.83, p=0.54) but was significantly associated with higher risk of HGPCa (i.e., Gleason score≥7 prostate cancer) (OR=2.31, p=0.03). For men with a known family history of PCa, the risks of PCa and HGPCa increased rapidly as BMI increased (PCa: OR=3.73, p=0.02; HGPCa: OR=7.95, p=0.002). The bmiPCPTRC generally underestimated the risks of PCa and HGPCa. Conclusions BMI provided independently predictive information regarding risks of PCa and HGPCa, after adjusting for other risk factors. BMI, especially among men with a known family history of PCa, should be considered for inclusion in any clinical assessment of PCa risk and recommendations regarding prostate biopsy.

Comparison of prostate cancer detection rates between the Vienna nomogram and the 10-core biopsy protocol

Objective: To compare the Vienna nomogram and the 10-core prostate biopsy protocol regarding whether there is superiority in prostate cancer detection. Methods: Between January and December 2012, a total of 215 patients applying to our outpatient clinic with lower urinary tract symptoms were evaluated, prospectively. Patients with a prostate-specific antigen level of 2.5–10 ng/mL and/or suspicious digital rectal examination were included in the study. Exclusion criteria were determined as recent pelvic radiotherapy, lower urinary tract surgery, history of acute urinary retention, or indwelling urinary catheter. Biopsies were taken systematically with at least 10 cores considering prostate volume and patient age. According to Vienna nomogram, in patients requiring 6- or 8-core biopsies, tissue sampling was completed to 10 cores (our standard protocol), whereas in patients requiring more than 10 cores additional tissue sampling was performed. Results: After the determination of inclusion/exclusion criteria, 170 patients were enrolled in our study. The median (min–max) age, prostate-specific antigen value, and prostate volume were 65 (48–86) years, 7.6 ng/dL (2.5–10), and 55 cc (17–150), respectively. Prostate cancer was detected in 49 (28.8%) patients with transrectal ultrasound–guided prostate biopsy according to the Vienna nomogram. We found that our standard 10-core biopsy protocol would have diagnosed prostate cancer in 46 (27.1%) patients in the same study group showing no statistically significant difference (p > 0.005). Conclusion: The findings of this study suggest that considering cancer detection rates no statistically significant differences were found between both methods. Further prospective research in this aspect is needed to define the ultimate prostate biopsy protocol.
