Introduction Hysterectomy is the most common gynaecological operation worldwide. The objective of the study is to analyze the various routes of hysterectomy and its complications when the decision of route is based on using a prospective algorithm tree. Methodology . It is an observational study to analyze the route of hysterectomy based on using a prospective algorithm. The decision tree is based on pelvic pathology, uterine size, vaginal access, pelvic adhesion, competency of the surgeon, choice of the patient, and complication of different routes of hysterectomy. Data were collected from preoperative, intraoperative, and postoperative records. Demographic factors, indications, routes of hysterectomy, and complications were recorded and analyzed by using SPSS software version 22. Observation . Among the malignant or suspected malignant pathology groups, TAH was performed in 89 cases and TLH was performed in 3 cases. Among the benign disease groups, VH was performed in 137(38.2%) cases, TAH was performed in 118(32.9%) cases, and TLH was performed in 104 (28.9%) cases. Operative time and a number of blood transfusions were significantly less with VH ( p value < 0.0001 and 0.004) compared to abdominal and total laparoscopic hysterectomy. Postoperative complication such as fever was more with abdominal hysterectomy ( p -value<0.00001) compared to VH and TLH. Vaginal discharge was more with VH and TLH compared to TAH ( p value −0.004) and wound infection was more in the abdominal route ( p value 0.001). Conclusion The abdominal route was the route of choice for surgery in malignancy or suspected malignant pathology. In benign pathology, VH was the most common and preferable route of surgery. Complications were found to be minimal with vaginal hysterectomy.

Introduction: Hysterectomy is one of the most common surgeries performed for common gynaecological indications, but less frequently done for tumors of female genital tract. Abnormal uterine bleeding (AUB) is one of the most common indications for hysterectomy. Histopathology is essential to confirm these common causes, but also helps to identify rare causes and to rule out malignancy. This study aims to anaylse the age pattern, clinical indications, clinicopathological correlation of hysterectomy cases along with distribution, causes and histopathological pattern of endometrial lesions in AUB. Material and Methods: The present study is a retrospective study done at Trichy SRM medical college hospital with hysterectomies done between January 2018 to November 2019 been included in the study. Clinical and Histopathological details were recorded, analysed and compared. Results: Of the 826 cases, Total abdominal hysterectomy was the most common surgery. The age varied from 21 to 85 with fourth decade as the most common age. Fibroid uterus and AUB were the most common clinical indication. Histopathology revealed leiomyoma as the most common diagnosis. About 95.5% cases correlated with clinical diagnosis. Of the 258 AUB cases, 100 had pure endometrial pathology and 31 cases showed combined endometrial and other pathologies. Totally 76 cases of endometrial hyperplasia were identified and 65 presented with AUB. About 11.6% AUB patients had malignancy and endometrial carcinomacomprised of 3.9% cases. Conclusion: Histopathological evaluation is essential to substantiate the clinical findings, identify missed pathologies, establishing a definitive etiology thereby aiding effective treatment of the pathology.

All over the world hysterectomy is the most common surgery performed in women next only to caesarean section. It is the effective treatment option for many gynaecological conditions. It has significant associated complications. Aim and objective of this study was to analyze the indications, route of hysterectomy and associated complications. Method: A retrospective study was conducted from January 2017 to December 2018 in department of Obstetrics and Gynaecology at SS Institute of Medical sciences and Research Centre, Davangere. A total of 300 hysterectomy patients were studied and parameters like age, parity, indication of hysterectomy and the type of hysterectomy, complications during and after the surgery were collected from the records and analyzed. Results: A total of 300 hysterectomies were done during the study period. Out of this, 220 (73.6%) were abdominal, 68(22.6%) were vaginal and 12(3.8%) were laparoscopic hysterectomies. Most common indication was symptomatic fibroid uterus (47.3%) and majority of the patients (48.3%) were in the age group of 41 -50 years. Overall complication rate was 14.3 %. Conclusion:Although hysterectomy is the definitive treatment for many gynaecological conditions, it is not risk free. Hence indication should be carefully evaluated. Patients should be counselled properly before this major surgery.
