Retroperitoneoscopic Managementof Infected Cysts in Adult Polycystic Kidney Disease

Abstract: Conservative measures are the mainstay of therapy in adult polycystic kidney disease (APKD). Pain, infection and obstructive uropathy are the major indications for intervention. Chronic pain has been treated with narcotic analgesics, needle aspiration of dominant cysts, and open renal cyst decortication. Laparoscopic cyst decortication, by either transperitoneal or retroperitoneal access, is a new emerging option with similar efficacy to open surgery and less morbidity. Cyst infection in these patients respond… Show more

“…The retroperitoneoscopic approach offers various advantages over a transperitoneal one [24,27,29], including direct approach to kidney without having to mobilize the colon; no violation of peritoneal cavity; closer proximity to the conventional open approach; and avoidance of spillage into the peritoneum, which is particularly important when infection is suspected or anticipated.…”

“…Laparoscopic cyst decortication often is used in polycystic kidneys in case of pain or hypertension. It is not unusual to find infected cysts in such patients [23]. Nadu et al [24] reported the case of a patient who underwent a heart transplant and had Aspergillus-infected polycystic kidney.…”

“…As such, percutaneous cyst aspiration and laparoscopic cyst decortication and drainage have both been reported as successful treatment modalities in patients with simple and ADPKD renal cysts [7,20]. In the latter condition, infected cysts may be dicult to identify based on ultrasound or CT scan, due to the large number and heterogeneous appearance of the renal cysts.…”
