Retroperitoneal Lymphadenectomy for Clinical Stage a Testis Cancer (1965 to 1989): Modifications of Technique and Impact on Ejaculation

Abstract: Results with primary retroperitoneal lymphadenectomy in 464 patients with clinical stage A nonseminomatous germ cell testis cancer (1965 to 1989) were reviewed. The false-negative staging error by clinical methods remains at 30%. The relapse rate in pathological stage A cancer patients was 11% (37 of 323), with 2 deaths. For pathological stage B disease 64% of the patients were cured by retroperitoneal lymphadenectomy alone. With modern adjuvant chemotherapy no stage B tumor relapsed since 1979 and the surviva… Show more

“…Such patients are typically offered 'adjuvant' chemotherapy in the form of two cycles of BEP with subsequent recurrences uncommon indeed (Donohue et al, 1993;Spermon et al, 2002;Albers et al, 2003). Patients without evidence of nodal metastases demonstrate relapse rates between 10 and 13%, the vast majority of which are pulmonary in nature and occur within the first year.…”

“…Nerve-sparing RLND (Jewett et al, 1988;Donohue et al, 1993) is a reasonable alternative to surveillance and does not usually lead to long-lasting sequelae. The operation represents, however, major surgery with 6-8 weeks' sick leave in most patients.…”

“…Following the failure of drug substitutions to convincingly reduce the toxicity of ACT, the next step was clearly to examine a single cycle of full dose (etoposide total dose 500 mg/sqM) BEP [58][59][60][61][62]. Very encouraging results were reported in the following two European studies:…”
