Retroperitoneal Cyst: An Uncommon Presentation of Filariasis

Abstract: Primary retroperitoneal parasitic cysts are rare. Here we report about a middle aged male patient from rural north India with a recent onset of central abdominal retroperitoneal lump, pain, and fever. After surgical resection due to diagnostic uncertainty, at histopathology, it turned out be a filarial cyst. After receiving a course of diethylcarbamazine, the patient is asymptomatic at 4 months' follow-up.

“…Additionally, clear well-defined margins allow for resection with ease. Therefore, if confirmed by preoperative investigations such as FNAC, treatment with DEC can be started first and depending on the clinical response, surgical excision may be reserved for persistent symptomatic cysts 6. In our case, our patient was not diagnosed with filariasis preoperatively and received various non-surgical modalities of treatment which failed to bring any improvement, therefore, a decision for surgical exploration was taken in view of long symptomatic period and well-defined planes on CECT.…”

“…Wuchereria bancrofti is the common causative agent of filarial infections in India. [3] Here, we present a rare case of filariasis presenting as a primary retroperitoneal cyst. The imaging findings are discussed in the case report, which leads us to the diagnosis with further confirmation on microscopy by the presence of microfilariae within the cyst.…”

“…The question of the occurrence of extraorganic retroperitoneal cysts (EORC), despite the increase in their incidence in recent decades, is poorly understood and is addressed only by single researchers, who describe mainly single observations of this pathology [1,2,3,4,5]. The lack of a unified classification of the EORC lies at the heart of a methodological problem that leaves a number of issues unresolved.…”
