Retrograde Ejaculation—a Commonly Unspoken Aspect of Prostatectomy for Benign Prostatic Hypertrophy

Abstract: The vast majority of men undergoing prostatectomy experience permanent retrograde ejaculation (RE). While newer, selective techniques of prostatectomy result in lower rates of RE by preserving the innervation to the urethra, the vast majority of men undergoing the classical techniques experience permanent RE due to destruction of the natural mechanisms preventing backflow of the ejaculate into the urinary bladder. A review of online information available to men reveals a lack of information on the importance o… Show more

“…During a pandemic, patients are advised to reduce visits to doctors and hospitals; the abolition of planned operations is required, so urological surgeries have to be prioritised according to their urgency and adapt to local resources [20]. Many different surgical interventions remove a part of the prostate gland; and after all of them, men may experience the loss of antegrade ejaculation, orgasmic dysfunction, or even erectile dysfunction with possible thermal effects on the neurovascular bundles [21], and this should not be simplified as it is a serious adverse effect [22].…”

“…With it, the need to adequately identify, assess and treat male sexual dysfunction becomes an important aspect to quality of life [13]. Despite the discussed scarcity in literature on the topic, it is agreed upon that men who consider antegrade ejaculation an important part of sexual pleasure and well being are likely to experience BPH surgery side effects very differently from those who are not sexually active or believe changes in ejaculation will not significantly change their pleasure [14]. Retrograde ejaculation is the most common event after conventional HOLEP surgery, and while it was not associated with decreased overall satisfaction with sex life in a retrospective cohort of over 2000 men, those who felt bothered by retrograde ejaculation reported impaired satisfaction with their sex life [15].…”
