Introduction Obesity complicated with climacteric syndrome (OCCS) is a disease in which estrogen levels are reduced, resulting in an imbalance between lipid metabolism, energy intake and consumption, and weight gain. Moxibustion, as non-interventional external treatment that prevents and treats diseases through thermal stimulation of relevant acupoints, has a certain advantage in the treatment of OCCS because of its affordability, convenience, efficacy, simplicity and high compliance.Consequently, we intend to carry out a comprehensive meta-analysis and systematic review on the effectiveness and safety of moxibustion for OCCS in order to contribute evidence for clinical trials. Methods and analysis From conception to December 2023, electronic searches will be conducted without language restrictions in the databases of CBM, VIP, CNKI, Wanfang, PubMed, Medline, EMBASE, and the Cochrane Library. The International Prospective Register of Systematic Reviews (PROSPERO), the Chinese Clinical Trial Registry Center, and Clinical studies will all be searched for continuing experiments.The eligible randomized controlled trials (RCTs) of moxibustion in treating OCCS should be included. The process of selecting studies, extracting data, and evaluating risk bias will be carried out separately by two reviewers. The meta-analysis of the included articles will be carried out via RevMan V.5.3 software. Meanwhile, the evidence's quality will be assessed by the Grading of Recommendations Assessment, Development and Evaluation application. Ethics and dissemination Since this is a systematic review and meta-analysis of already published studies that excludes patient information or privacy, ethics approval is not necessary. Background Obesity is an increasing problem worldwide. The effective treatments for obesity mainly include diet, physical activity, behavioral intervention, pharmacotherapy, and bariatric surgery, which all have certain limitations. As a specific type of acupuncture therapy, acupoint catgut embedding (ACE) has gained substantial attention in the management of obesity in recent years. Previous studies suggested that ACE may be an effective obesity treatment. However, the evidence for the efficacy of ACE in abdominal obesity (AO) remains inadequate due to the paucity of high-quality studies. Objective This study aims to investigate the difference in the effectiveness of catgut embedding at acupoints and catgut embedding at nonacupoints in patients with AO and to further validate the efficacy and safety of ACE for AO. Methods This is a multicenter, double-blind, 16-week randomized controlled trial. A total of 92 eligible participants with AO will be randomly divided into 2 groups (1:1 allocation ratio). The ACE group will receive catgut embedding at acupoints and the control group will receive catgut embedding at nonacupoints. The intervention will be performed every 2 weeks for a total of 6 sessions. Follow-up will be performed every 2 weeks for a total of 2 visits. The primary outcome is waist circumference. Secondary outcomes include body weight, BMI, hip circumference, and the visual analog scale of appetite. Upon the completion of the trial, we will evaluate the effect of catgut embedding at acupoints or nonacupoints on obesity indicators in patients with AO. For treatment outcomes, an intention-to-treat analysis will be performed. Results The start of recruitment began in August 2019 and is expected to end in September 2023. Conclusions Although studies have been conducted to demonstrate the effectiveness of ACE in the treatment of obesity, the evidence for the efficacy of ACE in AO remains insufficient due to the quality of the studies. This rigorous normative randomized controlled trial will verify the effect of catgut embedding at acupoints or nonacupoints in patients with AO. The findings will provide credible evidence as to whether ACE is an effective and safe treatment for AO. Background. In the past, moxibustion has been widely used to treat endocrine system disorders, but evidence of its effectiveness is scarce at this point. The aim of this multicenter, randomized, controlled trial is to evaluate the efficacy and safety of treating menopausal obesity with moxibustion. Methods/Design. There are six centers taking part in this randomized, controlled, parallel trial. A total of 216 patients with menopausal obesity will be randomly divided into two equal groups: the “moxibustion for harmonization of Yin and Yang” group and the gentle moxibustion group. A 12-week study period with moxibustion will be preceded by a 1-week baseline, followed by a 12-week follow-up. We will conduct an interim analysis to determine whether or not the treatment is efficacious and safe after 216 participants have completed a 12-week treatment period. Evaluations will be conducted at weeks 0, 4, 8, 12, 18, and 24. The main outcome is waist circumference (WC), and the rate of WC reduction will be compared to the baseline. An intention-to-treat analysis will be performed with a two-sided P value of <0.05 considered significant. Participants who withdraw from the trial will be analyzed according to the intention-to-treat formula (ITT). Discussion. These results will be used to support selecting the right moxibustion prescription and guiding the improvement of clinical efficacy. This trial will provide convincing evidence of moxibustion’s effectiveness and safety in the treatment of obesity by “moxibustion for harmonization of Yin and Yang,” which will be conducive to the promotion and clinical application of the theory of “moxibustion for harmonization of Yin and Yang.” Trial Registration. This trial is registered with Clinical Trials.gov: NCT04943705 (registered on June 27, 2021).
