AIM: To investigate the ratio of spontaneous regression of retinopathy of prematurity (ROP) and to explore the possible relevant predictive factors. METHODS: A retrospective review of 405 infants who were diagnosed with ROP and mother during pregnancy were collected. Stage, zone, and duration of ROP were recorded. Statistical analysis was performed on 51 possible predictive factors. RESULTS: Totally 356 infants showed spontaneous regression. The incidence was 100%, 95.3%, and 22.7% in stage 1, 2, and 3, respectively. The 13.4% of the ROP with plus disease eventually resolved spontaneously. All affected eyes of aggressive posterior retinopathy of prematurity (AP-ROP) failed to spontaneously regress. The mean duration of ROP was 7.2wk in patients with spontaneous resolution of ROP. Days of mechanical ventilation (OR=0.981, 95%CI, 0.965-0.997, P=0.021), retinal hemorrhage (OR=0.173, 95%CI, 0.064-0.470, P=0.001), delivery pattern (OR=2.750, 95%CI, 1.132-6.681, P=0.025), maternal anemia in pregnancy (OR=0.142, 95%CI, 0.036-0.563, P=0.005), the stages (at initial diagnosis OR=0.183, 95%CI, 0.041-0.816, P=0.026; at final diagnosis OR=0.031, 95%CI, 0.006-0.167, P<0.001), and with plus disease or not (OR=0.005, 95%CI, 0.001-0.031, P<0.001) were independent predictive factors of the spontaneous regression of ROP. CONCLUSION: Most mild ROP can spontaneously resolve. Active treatment is still recommended for stage 3 ROP, zone I ROP, AP-ROP, and ROP with plus disease. Prolonged mechanical ventilation and concurrent retinal hemorrhage reduce the likelihood of spontaneous ROP resolution. The pattern of delivery and the mother’s anemia during pregnancy can also affect the prognosis of ROP.

Impact of premature rupture of membranes on clinical outcomes of extremely premature infants: A propensity score matching study

BackgroundPremature rupture of membranes (PROM) is a common cause of extremely premature infants (EPIs) and also leads to adverse preterm complications. However, the effect of PROM on EPIs remains contradictory. This study used propensity score matching (PSM) to adjust the baseline characteristics to explore the impact of PROM on clinical outcomes of extremely premature infants (EPIs).MethodsMedical data of 470 EPIs at gestational age < 28weeks who received prenatal examination in our hospital between January 1, 2015 and December 31, 2020 were analyzed retrospectively. According to the presence or absence of PROM, they were divided into a PROM group and a non-PROM group. Ten covariates including birth weight, male sex, artificial conception, cesarean delivery, 5-min Apgar score ≤ 7, oligohydramnios, gestational hypertension, preeclampsia, antenatal steroid use, and complete steroid treatment were matched 1:1 by PSM. The major complication occurrence and mortality during hospitalization were compared between the two groups by t-test, nonparametric test or x2 test.ResultsAmong the 470 infants enrolled, 157 (33.4%) were in the PROM group and 313 in the no-PROM group. After matching the ten confounding factors,276 cases were successfully enrolled. The incidence of early pulmonary hypertension (EPH) and severe retinopathy of prematurity (ROP) in the PROM group were higher than those in the no-PROM group [44.2% (61/138) vs. 29.0% (40/138); 34.8% (48/138) vs. 21.7% (30/138), x2 = 6.886 and 5.790, both P < 0.05]. However, there was no significant difference in the in-hospital mortality and the incidence of other major complications between the two groups (all P > 0.05).ConclusionsPROM increased the incidence of EPH and severe ROP in EPI, but had no significant impact on in-hospital mortality, length of hospital stay, and the incidence of other complications.

Objetivo: analisar publicações científicas sobre a relevância dos fatores de risco maternos para a predisposição ao desenvolvimento de Retinopatia da Prematuridade (ROP) em recém-nascidos prematuros. Materiais e Métodos: trata-se de uma revisão integrativa da literatura. A pergunta norteadora definida para a investigação foi: “Quais são os fatores de risco maternos associados ao desenvolvimento da retinopatia da prematuridade?”. A busca dos artigos científicos ocorreu através das bases de dados Biblioteca Virtual de Saúde (BVS) e PubMed (U.S. National Library of Medicine), aplicando-se os descritores “Retinopathy of Prematurity” AND “Risk Factors”; e através da base Scientific Electronic Library Online (Scielo) aplicando-se “Retinopathy of Prematurity” AND “Risk”. Resultados: parte dos artigos (n=5) foi publicada em 2021. Os estudos foram realizados no continente asiático (n= 6), América do Norte (n=2), Europa (n=1) e África (n=1). Todas as pesquisas ocorreram em ambiente hospitalar, tendo em comum a abordagem de fatores relacionados aos recém-nascidos, mas também de fatores maternos, que consiste no foco deste estudo de revisão. Em relação aos fatores de risco maternos, houve destaque da correlação entre comorbidades das mães e o desenvolvimento de ROP, mas também foram abordados fatores nutricionais e sociodemográficos. Conclusão: as características, complicações, comorbidades e alterações ligadas ao feto/lactente, especialmente, baixo peso ao nascer e menor idade gestacional parecem ser mais relevantes para a ocorrência de ROP do que fatores associados à mãe.
