Lens-sparing pars plana vitrectomy (PPV) is often followed by cataract development. However, there have been few prospective studies evaluating the timing of cataract progression and potential associated factors. This was an observational study conducted at the Ophthalmology Unit of the University Hospital of Parma (Parma, Italy). Patients presenting with rhegmatogenous retinal detachment (RRD), who underwent PPV with preservation of the lens, were examined according to a scheduled follow-up (3, 6 and 12 months after PPV) and then preoperatively when cataract extraction surgery (CES) was indicated, or at the end of the study follow-up period (May 2021). The primary outcome was the interval between PPV and CES indication (based on predefined refractive criteria). A total of 36 eyes of 36 patients (mean age: 52 ± 10 years) were included in the study. Nineteen eyes (53%) were indicated for CES a median of 14.5 months (IQR: 12.0–24.8) after PPV. The nuclear and posterior subcapsular forms of cataract progressed significantly starting at 6 months after PPV. Older age at the time of PPV, silicone oil tamponade and RRD without macular involvement were significantly and independently associated with an earlier indication for CES. Patient age and the use of silicone oil tamponade must be taken into consideration when evaluating the risk of cataract development after PPV.

Rhegmatogenous retinal detachment (RRD) is a serious and emergency condition that may cause visual disturbance. Treatment includes pars plana vitrectomy with a tamponade such as intraocular gas or silicone oil (SO). In many countries, silicone oil is still favorable compared to intraocular gases as tamponade for reattachment of retinal detachment surgery. The application provides a higher anatomical success rate, especially in cases of proliferative vitreoretinopathy (PVR) that were previously considered untreatable. Objective assessment of the retinal nerve fiber layer (RNFL) using optical coherence tomography (OCT) in the eye with silicone oil tamponade is a challenge because of the limitations and difficulties in taking images. This study aims to assess the RNFL thickness changes in rhegmatogenous retinal detachment patients using SO tamponade and its subsequent removal conducted on a total of 35 post-operative RRD patients. Central macular and RNFL thickness, as well as best-corrected visual acuity (BCVA), were recorded at the time of tamponade and after the removal of the SO at 1, 4, and 8 weeks, respectively. The results showed that the changes in RNFL thickness significantly decreased in the group of ≤6 months, especially in the superior and temporal quadrants, and BCVA increased after SO removal (p < 0.05). Central macular thickness was significant (p < 0.001) at the end of the visit. Improved visual acuity is associated with decreased RNFL and central macular thickness after SO removal.

Purpose The current study utilizes microperimetry and optical coherence tomography angiography (OCTA) to assess the optic nerve head vasculature, retinal microvasculature, and retinal sensitivity before and after silicone oil (SO) removal. Methods This prospective observational case series study involved 30 eyes subjected to silicone oil endotamponade. Microperimetry and OCTA were utilized to assess the vascular density (VD) of the macula and optic nerve head, as well as the retinal sensitivity (RS), of the participants preoperatively and 1 month following SO removal. The correlation between the various parameters of OCTA and microperimetry was evaluated. Results There was a significant improvement in the postoperative best-corrected visual acuity (BCVA) (p-value < 0.001) and the postoperative total RS, which was 6.38 ± 2.34 dB as compared to a mean preoperative total RS of 5.04 ± 2.06 dB (p-value < 0.001) and showing a significant increase in all rings. However, there was no significant difference in the pre and postoperative macular VD. On the other hand, there was a significant increase in the postoperative VD of the whole disk and the peripapillary capillary plexus, p-values < 0.001 and 0.002, respectively. Conclusion The removal of SO resulted in significant improvements in retinal sensitivity, vision, and optic nerve perfusion. However, no significant change was observed in macular VD.
