Purpose To report the rate of retinal redetachment and intervention with combined removal of silicone oil plus internal search (ROSOplus) and to report the pathology identified. Methods Preoperative and peroperative findings were related to postoperative failure of the surgery defined as retinal redetachment postoperatively or silicone oil in situ at the final follow-up. Results Sixty-three patients were included in the study. Mean follow-up was 13 months. Retinopexy and further tamponade were used in 22 patients. Overall 'ROSO-plus' failed in 13 (21%) patients. Patients with subretinal fluid (SRF) in the inferior quadrants of the fundus during 'ROSO-plus' were particularly at risk of failure at 86% (six of seven patients) vs 12.5% (7/56) for the remainder (P ¼ 0.0002, relative risk ¼ 6.9, 95% confidence interval 3.2-14.6). The overall success rate at final follow-up (after any further surgery) for a flat retina without oil in situ was 83%. Conclusion The 'ROSO-plus' procedure allowed identification of problems expected to result in anatomical failure. Treatment did not prevent a high rate of postoperative retinal detachment. Refinement of the treatment algorithm is required with perhaps more use of silicone oil reinsertion in high-risk eyes.

Of the 168 eyes undergoing silicone oil removal from 1993 to 2000, 34 developed retinal detachment after the removal of silicone oil. The management and the results of the 34 eyes were reviewed. Surgical procedures included pars plana vitrectomy, encircling band, cryocoagulation, endotamponade. Five of the 34 eyes had a second retinal detachment after these operations. One of these 5 eyes had a third retinal detachment. Final retinal reattachment was achieved in all eyes. Visual acuity decreased in 16 of the 34 eyes with retinal detachment, and 18 of the 34 eyes had a visual acuity of less than 0.02. These results show that surgery for retinal detachment after silicone oil removal yields a high rate of anatomic success, while the visual outcome is poor for a substantial number of patients.

Zusammenfassung. Fragestellung: Trott laufender Verbesserungen der Technik in der PVR-Chirurgie (Proliferative Vitreoretinopathie) liegt die Reablatiorate nach Silikonolentfernung noch immer zwischen 9 and 20%. Wir haben daher unsere primare PVR-Operation standardisiert and in dieser Arbeit die Ergebnisse nach Silikonolentfernung bei Patienten mit rhegmatogener PVR untersucht. Methode: In einer retrospektiven Studie wurden 59 Patienten untersucht, die zwischen 1998 and 2001 wegen rhegmatogen bedingter PVR operiert wurden, and die nach kompletter Netzhautanlage eine Silikonolentfernung erhielten. Aphaken Patienten wurde bei dieser Operation gleichzeitig eine Linse implantiert. Beobachtungszeitraum war 6 Monate postoperativ. Ergebnisse: Von 59 Patienten wurde in 2 Fallen (3,4%) eine Reablatio beobachtet. 21 Patienten erhielten eine Silikonolentfernung kombiniert mit Linsenimplantation. 38 Patienten waren primar pseudophak. 70% der Patienten wiesen am Ende des Beobachtungszeitraumes einen Visus von 6/60 oder besser auf. Schlussfolgerung: Das Risiko einer Reablatio nach Silikonolentfernung bei rhegmatogener PVR wird entscheidend von den davor liegenden vitreoretinalen Eingriffen beeinflusst. Mit der Standardisierung der primaren PVR-Chirurgie konnen die Ergebnisse nach Silikonolentfernung verbessert werden. Abstract: Within the last decade the technique of PVR (proliferative vitreoretinopathy) surgery improved constantly. Surprisingly the retinal redatchement rate after silicone oil removal still remains the same and ranges from 9-20%. We standardized our primary PVR surgery and want to present in this study the outcome of silicone oil removal in these eyes.Methods: In a retrospective study we examined the files of 59 patients operated on between 1998 and 2001 suffering of rhegmatogenes PVR. In aphakic patient the silicone oil removal was combined with a posterior chamber lens implantation. The end of the observation period was 6 months postoperatively.Results: 2 of 59 (2%) patients developed retinal redetachment after silicone oil removal. 21 patients received a combined procedure with lens implantation. 38 patients were pseudophakic. 70% of the eyes regained a vision of 20/200 or better.Conclusion: The risc of retinal detachment after silicone oil removal is highly influenced by previous surgeries. We therefore believe that standardising the primary silicone oil surgery improves the outcome after silicone oil removal.
