Zusammenfassung ! Hintergrund: Ziel dieser Studie war es, die Streubreite der anatomischen Parameter des Ellenbogengelenkes zu untersuchen und zu analysieren, inwieweit diese anatomische Vielfalt von den gängigen Endoprothesen des Ellenbogengelenkes berücksichtigt wird. Methodik: Es wurden 46 Unter-und Oberarmknochen von erwachsenen Leichen untersucht (m/w 1,15:1) sowie 184 konventionelle Rçntgen-bilder von Ellenbogen vermessen (m/w 1,13:1) und mit den analogen Messwerten der Rçntgen-schablonen von 9 Prothesenmodellen verglichen. Das Durchschnittsalter in der anatomischen Gruppe betrug 76,9 (49 -96) Jahre und in der radiologischen Gruppe 56,4 Jahre (29 -100). Insgesamt wurden 85 Parameter definiert und in beiden Untersuchungsgruppen vermessen. Es konnten 19 Parameter an Rçntgenschablonen von 9 Ellenbogenprothesen analog vermessen und ausgewertet werden. Anhand acht dieser Parameter wurde die Passgenauigkeit einer Prothese ermittelt und anhand eines 3-Punkte-Scores bezüglich der Abdeckung der anatomischen Streubreite bewertet. Ergebnisse: Es zeigte sich ein signifikanter, geschlechtlicher Unterschied in den Messreihen beider Untersuchungsgruppen. Im Vergleich von rechten und linken Gelenken konnte kein signifikanter Unterschied festgestellt werden. Die Zuordnung der Messungen der Rçntgen-schablonen dieser Prothesen zeigt ohne Ausnahme, dass die Grçßen der Prothesentypen sich außerhalb oder im linken Schenkel der Verteilungskurve befinden. Die Prothese mit dem hçchsten Wert erreicht 63 % der Maximalpunktzahl für Passgenauigkeit. Der Winkel zwischen Schaft und Flexions-/Extensionsachse von Humerus und Ulna wird zum Teil nicht berücksich-tigt und beträgt physiologisch 4,5 respektive 8. Background: The aim of this anatomic study was to analyse the spread of anatomic parameters of the elbow and to examine how these parameters are considered in the design of the most commonly used elbow endoprostheses. Method: The bones of the humerus, ulna and radius of 46 adult cadavers (m/f 1.15:1) as well as 184 radiographs of the elbow (m/f 1.13:1) were analysed and anatomic parameters were compared to these of 9 elbow endoprostheses. The average age of the anatomic group was 76.9 (49 -96) years and of the radiological group 56.4 (29 -100) years. A total of 85 anatomic parameters were defined. For the 9 elbow endoprostheses 19 parameters were analysed and 8 parameters were used to assess the anatomic fit of the prostheses. Results: There was a significant difference in the gender of the patients but not on the side of the elbow. The anatomic parameters of the elbow prostheses were mostly out of or in the left side of the histogram. The endoprotheses with the best fit reached 63 % of the points for anatomic fitting. The angles between the axis of the shaft and the axis of motion are 4.5 for the humerus and 8 for the ulna and are not mentioned in most protheses.
