Background: South Africa is having difficulties in rolling out the National Health Insurance (NHI) policy. There are ongoing arguments on whether the NHI will provide access to quality and equitable healthcare it is intended to and whether South Africa is ready to implement the policy. Many stakeholders believe the country needs more preparation if the policy will be successful. Ghana, on the other hand, has successfully implemented the National Health Insurance Scheme(NHIS) for over 15 years. Objective: This paper sought to explore the implementation of the NHIS in Ghana and the lessons South Africa and other low-and middle-income countries can learn from such a process. Methods: A scoping review was conducted using the Joanna Brigs Institute's System for the Unified Management, Assessment and Review of Information (SUMARI) and Mendeley reference manager to manage the review process. Journal articles published on the NHIS in Ghana from January 2003 to December 2018 were searched from Science Direct, PubMed, Scopus, CINAHL, and Medline using the keywords: Ghana, Health, and Insurance. Results: The implementation of the NHIS has provided access to healthcare for the Ghanaian population, especially to poor and vulnerable. Despite the successful implementation of the NHIS in Ghana, the scheme is challenged with poor coverage; poor quality of care; corruption and ineffective governance; poor stakeholder participation; lack of clarity on concepts in the policy; intense political influence; and poor financing. Conclusion: The marked inequity in the South African health system makes the implementation of the NHI inevitable. The challenges experienced in the implementation of the NHIS in Ghana are not new to the South African healthcare system. South Africa must learn from the experiences of Ghana,a context that shares common socio-cultural and economic factors and disease burden,in order to successfully implement the NHI.

The use of telehealth to support, enhance or substitute traditional methods of delivering healthcare is becoming increasingly common in many specialties, such as stroke care, radiology and oncology. There is reason to believe that this approach remains underutilized within nephrology, which is somewhat surprising given the fact that nephrologists have always driven technological change in developing dialysis technology. Despite the obvious benefits that telehealth may provide, robust evidence remains lacking and many of the studies are anecdotal, limited to small numbers or without conclusive proof of benefit. More worryingly, quite a few studies report unexpected obstacles, pitfalls or patient dissatisfaction. However, with increasing global threats such as climate change and infectious disease, a change in approach to delivery of healthcare is needed. The current pandemic with coronavirus disease 2019 (COVID-19) has prompted the renal community to embrace telehealth to an unprecedented extent and at speed. In that sense the pandemic has already served as a disruptor, changed clinical practice and shown immense transformative potential. Here, we provide an update on current evidence and use of telehealth within various areas of nephrology globally, including the fields of dialysis, inpatient care, virtual consultation and patient empowerment. We also provide a brief primer on the use of artificial intelligence in this context and speculate about future implications. We also highlight legal aspects and pitfalls and discuss the ‘digital divide’ as a key concept that healthcare providers need to be mindful of when providing telemedicine-based approaches. Finally, we briefly discuss the immediate use of telenephrology at the onset of the COVID-19 pandemic. We hope to provide clinical nephrologists with an overview of what is currently available, as well as a glimpse into what may be expected in the future.

Zusammenfassung Hintergrund Wesentliche Ziele von Krankenhäusern sind eine hohe Qualität, Sicherheit, Effektivität und Effizienz der Leistungserbringung. Dies erfordert eine optimale Behandlungssteuerung durch das Fallmanagement auf den Krankenhausstationen (FM). Methoden Für eine Analyse der Mikro- und Makroprozesse des stationären Fallmanagements wurde ein strukturiertes Interviewinventar entwickelt. Anschließend wurde mit dem Interviewinventar eine Felduntersuchung als community based participatory research (CBPR) an homogenen Fokusgruppen von 24 deutschen, aus ökonomischer Sicht als Profitcenter wirkenden, öffentlichen Krankenhausabteilungen der Maximalversorgung durchgeführt. Ergebnisse Mit der CBPR-Feldstudie konnten 1.682 Mikroprozesse des stationären Fallmanagements der untersuchten Krankenhausabteilungen erfasst und beurteilt werden. Die Studie zeigt, dass alle Abteilungen zwar ein umfangreiches klinisches Fallmanagement betreiben, die Mikroprozesse des Fallmanagements allerdings durchgehend weder optimal zur Kostenminimierung noch maximal zur Liquiditäts-, Erlös- und Qualitätsverbesserung durchgeführt werden. Auch zeigt sich, dass die berufsgruppenübergreifende Durchführung der Prozesse des FM durch Ärzte und Pflegekräfte mit einer erheblichen Fragmentierung des FM verbunden ist. Schlussfolgerungen Die intensive Fragmentierung der Tätigkeiten des FM auf Krankenhausstationen legt die Erarbeitung von Strategien nahe, die Prozesse des stationären Fallmanagements zu homogenisieren.
