Results from 15 years of quality surveillance for a National Indigenous Point-of-Care Testing Program for diabetes

Abstract: HbA1c POC testing in QAAMS has remained analytically sound, matched the quality achieved by Australasian laboratories and met profession-derived analytical goals for 15years.

“…Some countries, such as Australia and New Zealand, have local guidelines on the use and implementation of POCT based on both national and international recommendations. 6 , 14 When testing for glucose, theoretical knowledge is required in order for the tester to be aware of factors such as haematocrit levels, systemic shock oxygenation status and exposure of strips to humidity, which can reduce the shelf-life of the strips. 15 , 16 A low haematocrit level (< 30% – 35%) may lead to overestimation of glucose, whilst a haematocrit above 45% may lead to underestimation of glucose results by some POCT devices.…”

“…The QAAMS network now operates in more than 220 sites across Australia, with sites conducting POC testing for hemoglobin A 1c for the diagnosis and management of diabetes and urine albumin to creatinine ratio for the early detection of renal disease. 16,17 Maps showing the general locations of sites participating in the Northern Territory, TTANGO, syphilis, and QAAMS POC testing networks are shown in Figure 1.…”

“…The Australian Government approved the use of the HbA1c test for diagnosis of the Indigenous Australian population enrolled exclusively in the QAAMS POC Testing Program. This decision was based on the consistently high analytical quality of POC HbA1c testing in QAAMS, as evidenced by the results of continuing external quality assurance and quality control testing over the past 15 years (46). DCA Vantage measured HbA1c values close to 6.5% both accurately and precisely.…”

“…DCA Vantage measured HbA1c values close to 6.5% both accurately and precisely. The authors argue that POC testing devices are suitable for the diagnosis of diabetes, and each individual POC device should be assessed independently when considering its suitability for diabetes diagnosis (46). Conversely, POC HbA1c testing may be used for screening purposes.…”
