Results after mitral valve replacement with mechanical prostheses in young children

Abstract: Mortality and repeated valve replacement are common after mitral valve replacement in children younger than 8 years, especially younger patients with significantly oversized valves. At valve reoperation, larger prostheses could be implanted, suggesting continued annular growth.

“…595 Mechanical prosthetic heart valves may be surgically placed in infants and children with congenital or acquired heart disease when their innate valve is not surgically repairable. 596 Thrombotic complications associated with mechanical prosthetic heart valves are well described in adults. For this reason, clinicians generally use VKAs to prevent complications, which include TE, valve thrombosis, and ischemic stroke.…”

“…These include postoperative LVOT obstructions, impairment of prosthetic valve mobility, and damage to the conduction system, which may cause complete AV block after surgery. The rate of permanent pacemaker (PPM) insertion has been reported to be 5-16% [3,5,7,12,19,21]. Another potential problem is damage to the left circumflex coronary artery, which can lead to postoperative heart failure [7,12,19].…”

“…Data from a number of studies indicates that 45-70% of infant MVR patients show freedom from reoperation at 10 years [2,3,8,11,12,18,19,21]. Normal growth in an infant patient often creates the need for reoperation.…”
