Restoration of Thrombosed Brescia-Cimino Dialysis Fistulas by Using Percutaneous Transluminal Angioplasty

Abstract: High anatomic success and excellent clinical patency can be achieved in the salvage of thrombosed arteriovenous fistulas. Percutaneous restoration of arteriovenous fistulas should be attempted before surgical recreation to optimize outcome in patients undergoing hemodialysis.

“…In other studies, urokinase or r-tPA (recombinant tissue plasminogen activator) were used in combination with PTA. The treatment results were similar to those presented in the studies where only PTA was used: PP 18-70%, SP -63-81% [20][21][22][23]. Vascular access through femoral artery and the use of long sheaths to manage AVF may not be widely utilized, but has clear advantages.…”

“…5000 IU heparin should be administered during the intervention. As a result of thrombus fragmentation via conventional PTA, Liang and colleagues achieved a direct success rate of 93 % with a patency rate of 70 % after one year [75]. Residual thromboses were additionally treated via urokinase.…”

“…Thrombectomy procedures include thrombus aspiration using a vascular sheath with a removable valve 15 , thrombectomy using a thrombus aspiration catheter or large-bore angiographic catheter 15,20 , thrombolysis using a thrombolysis catheter 32, 69, I-1, J-2 , or the so-called lyse-and-wait-procedure, in which urokinase is injected directly into the graft. 5,51,60,70,74,77 In cases in which the above procedures cannot be performed, such as when there are small remnant thrombi, a Fogarty catheter with a guidewire lumen can also be used. 40 In facilities that are able to perform surgical and catheter combined procedures, surgical thrombectomy using a Fogarty catheter and interventional therapy with simultaneous stenosis dilation is another treatment option.…”
