Restless sleep disorder in children: a pilot study on a tentative new diagnostic category

Abstract: We have characterized clinically and polysomnographically children with RSD and attempted a new diagnostic category. We also have identified an association between RDS and iron deficiency. Future larger studies are needed to confirm these findings and evaluate the natural progression of restless sleepers.

“…In recent years, the association between iron deficiency and sleep disorders has received growing attention in the scientific literature [ 155 ]. Low levels of iron have been associated with restless legs syndrome/periodic limb movement disorder (RLS/PLMD) [ 156 ] and RLS-induced insomnia [ 157 , 158 ], parasomnias [ 159 ], sleep disordered breathing [ 160 ], attention deficit and hyperactivity disorder (ADHD) [ 161 ], and “restless sleep disorder”, a newly proposed diagnosis [ 162 ].…”

“…One of our patients had coexistence of RSD with growing pains. Our previous research has shown that children with RSD do not have increased leg motor activation or discomfort (DelRosso et al., ); therefore, we do not have a suspicion of an increased overlap between growing pains and RSD but this might be a topic for future research, if the association proves to be significant.…”

“…Although RLS has inherent diagnostic challenges in young children (Picchietti & Stevens, ), video polysomnography has shown a difference in movement patterns in these two groups. Particularly, children with RLS have increased leg movements whereas children with RSD present with large body movements and repositioning (DelRosso et al., ). Although growing pains are not included in the ICSD‐3 sleep diagnosis, we routinely screen for growing pains as the literature has shown an overlap between symptoms of growing pains and RLS (Walters, Gabelia, & Frauscher, ).…”

“…Restless sleep in children has been associated with the development of RLS later in life. 63 Recently, a new entity of restless sleep disorder has been proposed, 64 possibly preceding the development of RLS. Possible RLS: The child is observed to have behavior manifestation of lower extremity discomfort when sitting or lying, accompanied by motor movement of the affected limbs.…”
