Home-based or remote exercise testing in chronic respiratory disease, during the COVID-19 pandemic and beyond: A rapid review

Objectives: To identify exercise tests that are suitable for home-based or remote administration in people with chronic lung disease. Methods: Rapid review of studies that reported home-based or remote administration of an exercise test in people with chronic lung disease, and studies reporting their clinimetric (measurement) properties. Results: 84 studies were included. Tests used at home were the 6-minute walk test (6MWT, two studies), sit-to-stand tests (STS, five studies), Timed Up and Go (TUG, 4 studies) and step tests (two studies). Exercise tests administered remotely were the 6MWT (two studies) and step test (one study). Compared to centre-based testing the 6MWT distance was similar when performed outdoors but shorter when performed at home (two studies). The STS, TUG and step tests were feasible, reliable (intra-class correlation coefficients >0.80), valid (concurrent and known groups validity) and moderately responsive to pulmonary rehabilitation (medium effect sizes). These tests elicited less desaturation than the 6MWT, and validated methods to prescribe exercise were not reported. Discussion: The STS, step and TUG tests can be performed at home, but do not accurately document desaturation with walking or allow exercise prescription. Patients at risk of desaturation should be prioritised for centre-based exercise testing when this is available.

Objectives: to evaluate the concurrent validity of the six-minute step test (6MST) in assessing exercise capacity of COPD patients using the six-minute walk test (6MWT) as a gold-standard. The predictive validity of the 6MST was assessed to determine a cut-off point for identification of low exercise capacity. Method: thirty-two COPD patients (50-87 years old) with mild to very severe obstruction performed the 6MST and 6MWT twice. Results:Concurrent validity: a strong positive correlation (Pearson) between the number of ascents on the first (T1), second (T2) and the best of both (T1 or T2) tests during the 6MWT was observed. Although a moderate negative correlation with BODE index and FEV1 was found, it was considered insufficient to test the validity, therefore ROC curves were not applied. The predictive validity (ROC) of the 6MST to identify low physical capacity (compared with the 6MWT) using the performance of T1 or T2, or solely T1 was considered accurate, and the area under the curve was 0.8 (IC95% 0.62-0.98) and 0.85 (IC95% 0.70-0.99), respectively. To classify patients, the cut-off points of 86 and 78 steps were chosen, with both values showing 90% of sensitivity and specificity of 64% and 68% for T1 or T2, or solely T1, respectively. Conclusion: The number of steps on the 6MST was valid to verify exercise capacity in COPD patients and the cut-off point of 78 steps was able to identify patients with poor exercise tolerance. Values under this cut-off point are considered to identify patients with a poorer prognosis.

Respostas cardiorrespiratórias durante dois testes de exercício submáximos em participantes de um programa de reabilitação cardíaca: resultados preliminares RESUMO Introdução-A capacidade submáxima de exercício avalia a capacidade funcional do indivíduo, podendo ser mensurada pelo Teste de Caminhada de Seis Minutos (TC6m) e pelo Teste do Degrau de Quatro Minutos (TD4). Objetivo-Comparar as respostas cardiorrespiratórias e percepção de esforço entre o TC6m e o TD4 em pacientes submetidos à cirurgia de revascularização do miocárdio (CRM) participantes de um programa de reabilitação cardíaca. Metodologia-Estudo transversal composto por sete homens (58,71 ± 9,37 anos). Foram analisadas as respostas cardiorrespiratórias (saturação periférica de oxigênio-SpO2; frequência cardíaca-FC; pressão arterial sistólica-PAS; pressão arterial diastólica-PAD), sensação de dispneia (SD) e sensação de fadiga de membros inferiores (SFMMII) por meio da Escala de Borg modificada. Os testes seguiram as diretrizes da American Thoracic Society (ATS 2002),). Resultados-Constatou-se diferença significativa (p<0,05) nos deltas da PAS e PAD entre os testes. Conclusão-Nossos resultados sugerem que tanto o TC6m quanto o TD4 estimam a capacidade submáxima de exercício, porém o TD4 exige uma maior demanda metabólica, observada por maior incremento da pressão arterial.
