Responsiveness and interpretability of incontinence severity scores and FIQL in patients with fecal incontinence: a secondary analysis from a randomized controlled trial

Abstract: The instruments available to date to evaluate severity and quality of life in FI do not yet attain the highest levels of psychometric soundness. As the focus of patients may differ from that of physicians, it is recommended that several measures should be included for evaluation. So far, there are suggestions that the Wexner score is most suitable for severity assessment and the FIQL for evaluating quality of life.

“…27 Experiencing all AI symptoms, however, was the only symptom or combination of AI symptoms found to have a profound impact on all four FIQL domains, also shown previously. [26][27][28] The participants in previous studies tended to be older women with long-standing AI symptoms and are thus not directly comparable to our group of young healthy primiparae. The poor association between AI symptoms and 'Depression' in this study may reflect that the young expectant mothers were possibly more focused on their imminent or recent delivery and their new role as mothers, rather than on the inconveniences possibly thought to be pregnancy-related and of short duration.…”

“…24 The study population is relatively large and has a similar response rate to previous studies. 3,13,25 The validity of the two assessment tools has been documented previously 20,23,[26][27][28][29] ; however we only used some of the items derived from the St. Mark's score, rather than the validated total score. This may have influenced the results, and our findings must therefore be interpreted with caution.…”

“…Because FI is a major stressor in life, the Fecal Incontinence QoL scale is a condition specific QoL questionnaire and is commonly used for the assessment of ARS [22]. All of these scoring systems are very useful for assessing FI, but a meta-analysis of the several studies was not possible due to the different scorings scales.…”

“…Every serious sphincter injury also requires the grading of the quality of life sooner or later after the injury (FIQL) (6). That is why the reparation of an anal sphincter injury and the resulting fecal incontinence (if present) is essential for the injured patient and his/her family, as well as for the wider community because the physical and psychological recovery affects the working ability and the economic cost of the care.…”
