Introduction: Isolated teratozoospermia is rarely encountered in patients with clinically palpable varicocele, and the consequences of varicocelectomy remain unclear in these cases. In the current study, we assessed the effect of varicocelectomy on sperm morphology in a series of patients with infertility. Materials and Methods: The data obtained from 52 primary subfertile men with isolated teratozoospermia and clinical palpable varicocele were reviewed retrospectively. Varicocelectomy was performed on 29 patients, and the remaining 23 patients did not undergo any treatment (control group). The differences in sperm morphology assessed by using Kruger’s strict criteria 3 months before and 12 months after varicocelectomy were taken into consideration for statistical analysis. Results: One patient who underwent varicocelectomy was excluded from the study due to varicocele recurrence. Thus, data obtained from the remaining 28 patients were evaluated. Following varicocelectomy, significant improvement was detected in the ratio of normal sperm forms (p < 0.001). While there was significant improvement in the ratio of sperm cells with head defects, tail defects and immature forms (p < 0.001), the ratio of sperm with acrosome and mid-piece defects was not changed (p > 0.05). While an overall spontaneous pregnancy was achieved in 5 of the 28 (17.8%) couples in the varicocelectomy group within 12 months after operation, neither improvement in sperm morphology nor pregnancy in the patients’ partners was detected in the control group. Conclusion: Varicocelectomy caused a significant improvement in sperm morphology, particularly in immature forms and forms with head and tail defects evaluated by Kruger’s classification.

Вплив гепатомаксу на фертильність чоловіків ВСТУпЗ'явилося багато повідомлень про зниження кількісних та якісних характеристик сперми та збільшення випадків патології чоловічої статевої сфери наприкінці ХХ -початку ХХІ сторіч [1,9]. Відсоток позитивних ре-зультатів у лікуванні чоловічого безпліддя стабілізувався на рівні 16-32 % [14]. Така низька ефективність є наслідком того, що до теперішнього часу недостатньо вивчено при-чини чоловічої ін-та субфертильності. Для розробки правильної тактики та оптимальних схем лікування необхідна верифікація різ-номанітних факторів чоловічого безпліддя. Від точності встановлення його причини залежить подальше вивчення патогенезу порушення репродуктивної функції, що буде сприяти розробці ефективних методів терапії.поряд з причинами, котрі порушують репродуктивну здатність -і є складовою екскреторного безпліддя, існують причини секреторного безпліддя, серед яких запаль-ні захворювання репродуктивної системи, викликані урогенітальними інфекціями, і становлять від 8,5 до 38,4 % [14, 20].Морфологічні зміни у гонадах, що вини-кають внаслідок запальних процесів приз-водять й до функціональних змін. Останні характеризуються порушеннями багатосту-пінчастої системи утворення гамет та їх життєзабезпечення, яка включає механізм поділу клітин сім'яного епітелію, їх диферен-ціювання, матурації, селекції, антиімунного, осмотичного та антиоксидантного захисту (АОЗ) сперматозоїдів.Якщо клас патологічних варіантів пору-шення репродуктивної функції у чоловіків має диференційне лікувально-діагностичне забезпечення у рамках секреторно-екскре-торної концепції їх класифікації, то клас функціональних варіантів не знайшов сво-ISSN 0201-8489 Фізіол. журн., 2013, Т. 59, № 3 120 го застосування в практичній медицині, у зв'язку з відсутністю діагностичних мар-керів для їх виділення. Вони трактуються як "ідіопатичні" і при цьому застосовується емпіричне лікування. Коректно виконані до-слідження інституту епідеміології Лідського університету, показали, що останнє зазвичай, призводить до погіршення запліднювальної здатності еякуляту.Вивчення особливостей морфотипу спер-матозоїдів у чоловіків з безпліддям виявляє певну мономодальність в його відхиленнях. Це дає змогу зробити припущення про те, що зміни нейрогормонального контролю різного рівня розвитку гамет залишає свій "відбиток" на їх морфотипі залежно від періоду сперма-тоутворення.Запальні захворювання репродуктивної системи, які викликані урогенітальними ін-фекціями супроводжуються активацією білих кров'яних клітин сперми, які вивільнють протеолітичні ензими, цитокіни та реактив-ні кисневі сполуки (РКс) [19]. Негативна асоціація між надмірною продукцією РКс і чоловічою фертильністю продемонстрована в деяких дослідженнях [10,12,13, 15,18]. Varicocele is associated with an oxidative stress condition. We have measured the antioxidant defenses of varicocele patients both at the local (seminal plasma) and systemic (blood plasma) levels. The antioxidant defenses, as evaluated by the total reactive antioxidant potential parameter, decrease both in the seminal (controls = 676 ± 128; patients = 386 ± 186) and blood (controls = 519 ± 63; patients = 268 ± 110) plasma of varicocele patients. Compared with controls, patients with both normal spermiograms and spermiograms altered in motility or morphology demonstrated lower values. The results obtained suggest that varicocele‐associated oxidative stress is evidenced both at the local and systemic levels. This conclusion is supported by results showing that urinary spontaneous chemiluminescence is also significantly increased in the patients.

Abstract: Surgical correction of varicocele was associated with significant improvement in sperm morphology evaluated using Kruger classification. Concentration and count improved after varicocelectomy. Changes were observed as early as 3 months after surgery.
