Response of chronic and aggressive periodontitis to treatment

Abstract: Patients with aggressive periodontitis can be both rewarding and frustrating to treat in clinical practice. Interindividual variation in response to therapy can be widespread, and we do not clearly understand the reasons for this variable response. It is possible that new research into the resolution of inflammation may reveal basic differences between patients with chronic periodontitis and those with aggressive disease. In addition, future research involving modulation of host inflammatory responses may clar… Show more

“…6 Despite the many complications caused by periodontitis, no treatment protocol is known to be able to fully control this disease. Currently, a combination of mechanical treatments and systemic antibiotics are used as treatment 7 ; however, periodontal pathogens are not completely removed by the mechanical removal of the biofilm; and there are problems associated with the use of antibiotics, 9 and eventual complications related to antibiotics such as allergic reactions, gastrointestinal problems and bacterial resistance. 10 Therefore, researchers are trying to find alternative methods for treatment of periodontal diseases.…”

“…To retain teeth cannot help but complicate the treatment-planning process. Therefore, the patient with aggressive periodontitis to have experienced attachment loss would be expected at a younger age, at a faster rate and to a greater extent than the patient with chronic periodontitis (Deas et al, 2010).…”

“…Some reports support the existence of subgingival microbiota resistant to antibiotics of choice (33)(34)(35)(36)(37)(38)(39)(40) , which could explain eventual failures in the therapeutic modality. Localized aggressive periodontitis is mainly associated with the bacteria Aggregatibacter actinomycetemcomitans (41,42) , while generalized aggressive periodontitis is strongly associated with specific bacteria such as Porphyromonas gingivalis, Tannerella forsythia (43,44) and Aggregatibacter actinomycetemcomitans, Gram-negative coccobacillus, capnophile, microaerophilic (45) .…”
