Respiratory Syncytial Virus (RSV) is associated with most of the acute viral respiratory tract infections causing hospitalization with a peak during the first months of life. Many clinical trials of RSV vaccine candidates are being carried out. The aim of this study was to obtain epidemiologic information to give suggestions on target populations and prevention strategies before the introduction of new vaccines or monoclonal antibodies. We retrospectively evaluated, over a 5-year period (September 2014–August 2019), a population of hospitalized Italian children aged 0–6 years with a laboratory confirmed diagnosis of RSV infection. Risk factors, seasonality of RSV infection, distribution according to age, cases of coinfections and reinfections and cases needing Intensive Care Unit were evaluated. Hospitalizations due to RSV were 624 in the period under study. The peak was found between November and April, with 80.4% of cases recorded between December and February. 62.5% of cases were found in children under three months of age and 41% in children under 30 days old. The need for intensive care was associated with younger ages, with 70.9% of cases in children below three months of age. Unless the incoming vaccines demonstrate a strong herd protection effect, preventive strategies should be aimed at newborns or at maternal immunization.

The knowledge of the structural properties of molecules and of their recognition and interaction patterns with partners is a key feature for understanding their functional properties. This is particularly true for the comprehension of complex cellular processes in which several interacting proteins are involved. The structural characterization of all the molecules involved in a process allows the understanding of the malfunctioning of such process and eventually the design of biomole-[a] Magnetic Lucia Banci is a Professor of Chemistry at the University of Florence. She has extensive expertise and has provided original contributions and breakthroughs in Structural Biology and in biological NMR spectroscopy. She has addressed and unraveled many aspects of the biology of metal ions in biological systems. The innovative in cell NMR approach developed by Lucia Banci and her group allows for the detection of human individual proteins in living human cells with atomic level resolution. She also exploited the extensive knowledge of structural biology approaches through NMR expertise to develop an absolutely innovative approach to vaccine design, based on the knowledge of the structure of the pathogen antigens and of the interaction patterns with antibodies, to design structure-based vaccines. Francesca Cantini graduated in Chemistry in 2000 and received her PhD in Structural Biology in 2004 at the University of Florence. She is now a permanent research scientist at the University of Florence. Her research is focused on the structural and dynamic characterization of proteins and protein complexes through NMR spectroscopy. Since 2000, her research activity is dedicated to the comprehension of the molecular mechanisms responsible for metal trafficking and metallo-protein maturation. Since 2005, she collaborates with GSK Vaccines (Siena, Italy) on the structural characterization of vaccine candidates and their interaction with antibodies. Eur. J. Inorg. Quantitative PCR (qPCR) is widely used to detect viruses. However, mismatches occurring in the 3′-end of the primers reduce amplification efficiency of qPCR and limit its capacity in detection of highly variable viruses. Here, we reported a mismatch-tolerant RT-qPCR with a small amount of additional high-fidelity DNA polymerase for simultaneous detection of RSV-A and RSV-B. The novel assay had higher amplification efficiency for various variants forming mismatches with the primers than the conventional RT-qPCR, and showed good specificity and sensitivity. It demonstrated a good correlation coefficient with a commercial RSV detection kit and had relatively lower Ct values than the kit for 16 of 20 RSV-positive samples. The mismatch-tolerant qPCR technique is a promising approach for sensitive detection of highly variable viruses.
