A kötőszöveti betegségek tekintetében a pulmonalis manifesztáció megjelenése szisztémás lupus erythematosusban az egyik leggyakoribb. Lupusban bármely szervrendszer érintett lehet, mégis a tüdőérintettség az esetek döntő több-ségében kialakul, és jelentős hatással van a betegség lefolyására. Szisztémás lupus erythematosusban a pulmonalis manifesztációkat az anatómiai érintettség alapján öt csoportba lehet sorolni: pleura, tüdőparenchyma, bronchusok és bronchiolusok, pulmonalis vasculatura, illetve a légzőizmok érintettsége. A leggyakoribb pulmonalis érintettség a pleuritis, amely pleuralis folyadékkal vagy a nélkül is előfordulhat, gyakori a pulmonalis vascularis és a felső és alsó légúti érintettség, a tüdőparenchyma-eltérések és a diaphragmadiszfunkció (shrinking lung szindróma). A szerzők a közlemény kapcsán a lupusban szenvedő betegek pulmonalis eltéréseire, ezek diagnosztikájára, a kezelésükre és a prognózisra szeretnék felhívni a figyelmet. Orv. Although lupus has the potential to affect any organ, lung involvement is observed during the course of the disease in most cases and it is prognostic for outcome. Pulmonary manifestations in lupus can be classified into five groups based on the anatomical involvement: pleura, lung parenchyma, bronchi and bronchioli, lung vasculature and respiratory muscles can be involved. The most common respiratory manifestations attributable to lupus are pleuritis with or without pleural effusion, pulmonary vascular disease, upper and lower airway dysfunction, parenchymal disease, and diaphragmatic dysfunction (shrinking lung syndrome). In this article the authors summarize lung involvement of lupus, its diagnosis, therapy and prognosis. Lung involvement in rheumatologic diseases has a broad spectrum of clinical and radiological presentations, from acute and inflammatory predominance to chronic and fibrotic lung disorders. Therapy has to be guided by the type of rheumatologic disease, the kind of lung involvement and the risk associated with the clinical state of the patient and prognostic factors. Although several therapeutic approaches have been used, the best treatment is still not certain; clinical trials are mandatory, which would require a better knowledge of the pulmonary pathogenesis of immunologic diseases. The following review is focused on the therapeutic options for those rheumatologic disorders that could present as a predominant interstitial lung disease which may confer a bad or life-threatening prognosis.

Pulmonary hypertension is a rare and potentially life-threatening complication of Systemic lupus erythematosus (SLE), and 5 cases has been previously documented in pregnancy. Four cases died after delivery and only one case was alive. We describe the case of a 28-year-old pregnant woman with pulmonary hypertension related to SLE with no previous history of immunologic disease including SLE. Diagnosis was made at 22 weeks of gestation. Medication including prednisolone and hydroxychloroquinone was commenced immediately and continued throughout the pregnancy. On fetal sonogram, the fetal growth was 3~10 percentile and diastolic notch of uterine arteries was noted. However, a healthy baby girl weighing 2,400 g was born in planned vaginal delivery at gestation week 38. There were no postpartum complications. to 14% of patients with SLE. Overall mortality is 25~50% two years after diagnosis of pulmonary hypertension. 4 The most common symptoms of pulmonary hypertension in patients with SLE are fatigue, dyspnea and syncope. Raynaud's phenomena is present in 75% of patients with pulmonary hypertension related to SLE, compared with 25% in patients with SLE without pulmonary hypertension. 5 Echocardiography reveals right ventricular hypertrophy and dilatation.Pulmonary hypertension in patients with SLE is associated with intimal hyperplasia, smooth muscle hypertrophy and medial thickening, similar to the changes seen in primary pulmonary hypertension. 4 The development of pulmonary hypertension is not necessarily consistent with the stage of the SLE and severity of
