Respiratory Involvement in Patients with Neuromuscular Diseases: A Narrative Review

Abstract: Respiratory muscle weakness is a major cause of morbidity and mortality in patients with neuromuscular diseases (NMDs). Respiratory involvement in NMDs can manifest broadly, ranging from milder insufficiency that may affect only sleep initially to severe insufficiency that can be life threatening. Patients with neuromuscular diseases exhibit very often sleep-disordered breathing, which is frequently overlooked until symptoms become more severe leading to irreversible respiratory failure necessitating noninvasi… Show more

“…However, episodic attacks of acute respiratory failure (ARF) may further aggravate the already existed respiratory compromises (7). Factors posing a risk of ARF in children with NMD are usually multifactorial and occur simultaneously (8)(9)(10)(11). Table 1 summarizes the risk levels of various NMD potentially affected by the acute respiratory compromise.…”

Muscle weakness predisposes to repetitive pneumonia and atelectasis, leading ultimately to respiratory failure 1,2 . Clinical assessment of respiratory function in myopathic patients is a poor predictor of sleep-disordered breathing (SDB) and objective respiratory diagnostic tests are essential 2,3 .…”

“…Similarly, the institution of intensive mechanical respiratory and nutritional support reduced risk of death in spinal muscular atrophy (SMA) type 1 patients by 70% 9 . Despite the use of NIV, however, progressive respiratory insufficiency remains the main cause of morbidity and mortality for individuals with NMD 10 . While NIV has been a major advance for individuals with NMD, survival without dependence on respiratory technology would represent a huge improvement in quality of life.…”
