Respiratory failure due to diaphragm paralysis after brachial plexus injury diagnosed by point-of-care ultrasound

Abstract: A man in his fifties was injured in a traffic accident and diagnosed with traumatic subarachnoid haemorrhage, liver injury, and fractures of the rib, right clavicle, right scapula and right femur. He also presented with motor and sensory disturbances of the right upper extremity and was suspected of having a brachial plexus injury. After undergoing mechanical ventilation due to multiple traumas, he was extubated. However, he developed acute respiratory failure and required reintubation. Respiratory symptoms we… Show more

“…We found four case reports describing the identification of hemidiaphragmatic dysfunction with DUS. The underlying causes were phrenic nerve paralysis, dermatomyositis exacerbation, brachial plexus damage and amyotrophic lateral sclerosis, but diaphragm dysfunction assessment was the main finding that led to the correct diagnosis [63,[77][78][79].…”

“…Stroke can also be a cause of diaphragm paralysis, especially when involving the brain stem. Ultrasound assessment of diaphragm motility and thickness can be useful to diagnose this condition and monitor its evolution [ 169 , 170 , 171 ]. Respiratory symptom severity is generally related to the degree of dysfunction [ 169 , 170 , 171 ].…”

“…The position difference between diaphragmatic contraction and relaxation on diaphragm ultrasound (DUS) M-mode images is known as diaphragmatic excursion (DE) ( 32 , 33 ). DE may be abnormal in cases of diaphragmatic dysfunction, such as during spinal cord injury ( 34 , 35 ), muscular dystrophy ( 34 , 36 ), and diaphragmatic paralysis ( 34 , 37 ). Pneumothorax can also cause diaphragmatic dysfunction because high pleural pressure may push the diaphragm away from its normal position.…”
