Resources-Stratified Guidelines for Classical Hodgkin Lymphoma

Abstract: Hodgkin lymphoma is a haematological malignancy predominantly affecting young adults. Hodgkin lymphoma is a highly curable disease by current treatment standards. Latest treatment guidelines for Hodgkin lymphoma however imply access to diagnostic and treatment modalities that may not be available in settings with restricted healthcare resources. Considerable discrepancies in Hodgkin lymphoma patient survival exist, with poorer outcomes reported in resources-constrained settings. Resources-stratified guidelines… Show more

“…Bone marrow (BM) involvement is infrequent [7]. Correlation with EBV infection shows contrasting rates, ranging from 90-100% in Latin America, Africa, and Asia, to up to 50% in Western Europe and North America [8]. Doxorubicin, Bleomycin, Vinblastine, and Dacarbazine (ABVD) polychemotherapy and radiotherapy are the standard treatments for cHL [1,7,8] supported by autologous hematopoietic stem cell transplantation in at least 50% of patients [1].…”

“…Hodgkin lymphoma is a highly curable disease by current treatment standards. It implies access to diagnostic and treatment modalities that may not be available in settings with restricted health care resources [ 39 ]. A good score on this indicator therefore directly measures access to quality care.…”

“…3 PET-adapted therapies can be challenging to implement when timely access, interpretation, and reimbursement of PET/CT scans are limited. 11 We surveyed oncologists who treat patients with cHL as part of CONNECT (Classic Hodgkin Lymphoma: Real-World Observations from Physicians, Patients, and Caregivers on the Disease and Its Treatment), which is, to our knowledge, the first real-world survey of physicians, patients, and caregivers about cHL, to understand their treatment decision-making process and how PET/CT scan utilization and interpretation, and barriers to PET/CT scan access, reimbursement, and comprehension, influence treatment choices.…”
