Resources Involved in Managing Retinal Detachment Complicated by Proliferative Vitreoretinopathy

Abstract: Treatment of eyes that developed PVR after initial surgery cost significantly more than eyes with no PVR or established PVR (P < 0.01). Improvements in the management of RD aimed at preventing PVR and advances in PVR treatment may have significant financial as well as clinical benefits.

“…Patients who develop postoperative PVR (grade C or greater) double the costs of management in terms of higher number of surgical procedures and a more extensive follow-up period compared with patients without PVR (Patel et al, 2004). A recent study compared the outcomes of twenty-five gauge (25-G) surgery in RD complicated with grade C PVR with or without anterior PVR (Sato et al, 2014).…”

“…When calculated from insurance payments to Medicare beneficiaries, the cost associated with cystoid macular oedema after cataract surgery was reported to be 47% higher than those of controls,8 while the cost of managing cases with endophthamitis after cataract surgery was 1.45 times higher 9. In patients who have proliferative vitreoretinopathy there is a doubling of the cost of retinal detachment surgery 11. The additional costs incurred by inpatient treatment for endophthalmitis compared to outpatient treatment are significant,10 12 although inpatient treatment is not part of the management protocol for PCT.…”
