Patients with advanced cancer often experience debilitating anxiety symptoms that interfere with quality of life and relate to worse medical outcomes. Although cognitive behavioral therapy (CBT) is an empirically-validated, first-line treatment for anxiety disorders, clinical trials of CBT for anxiety typically exclude patients with medical comorbidities in general, and those with terminal illnesses, such as advanced cancer, in particular. Moreover, CBT has generally targeted unrealistic fears and worries in otherwise healthy individuals with clinically significant anxiety symptoms. Consequently, traditional CBT does not sufficiently address the cognitive components of anxiety in patients with cancer, especially negative thought patterns that are rational but nonetheless intrusive and distressing, such as concerns about pain, disability and death, as well as management of multiple stressors, changes in functional status and burdensome medical treatments. In this paper, we describe a treatment approach for tailoring CBT to the needs of this population. Three case examples of patients diagnosed with terminal lung cancer are presented to demonstrate the treatment methods along with outcome measures for anxiety and quality of life.

The purpose of this study was to assess the effect of a tailored cognitive-behavioral therapy (CBT) for depression and anxiety symptoms in Mexican terminal cancer patients. A non-concurrent multiple baseline design across individuals was used. Nine patients participated in the study, and they received four to six therapy sessions. The effect size of the intervention range (NAP and Tau indexes) in the nine patients indicated that the intervention had from weak to moderate impact for anxiety and depression symptoms in this population. Similarly, the overall standardized mean difference was also moderate, with a reduction of 0.54 and 0.76 standard deviations in depression and anxiety symptoms, respectively. This study provides initial evidence supporting the positive effect of CBT for patients with terminal cancer and with mood problems when facing their impending death.

El presente trabajo forma parte de la investigación doctoral del primer autor bajo la dirección del último. Resumen El objetivo de este trabajo es describir los principales componentes y características de la terapia cognitivo conductual cuando se utiliza para tratar problemas de ansiedad en pacientes con cáncer terminal. A lo largo del escrito se describen los cuatro componentes que más se han utilizados en las investigaciones en este área: 1) principios de la terapia cognitivo conductual 2) técnicas de relajación 3) identificación y reestructuración de pensamientos negativos 4) planeación de actividades. De igual forma, se describe el promedio de duración de la terapia cognitivo conductual, número de sesiones, intervalos de aplicación y los principales problemas reportados cuando se aplica la terapia. Finalmente, se especifican recomendaciones para la aplicación de la terapia y para el desarrollo de líneas futuras de investigación en el área.
