Resolution of Severe Sleep-Disordered Breathing with a Nasopharyngeal Obturator in 2 Cases of Nasopharyngeal Stenosis Complicating Uvulopalatopharyngoplasty

Abstract: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is characterized by episodic decrements in airflow due to upper airway obstruction. Uvulopalatopharyngoplasty (UPPP) is a potential therapy for OSAHS. Nasopharyngeal stenosis is a rare complication of UPPP that worsens OSAHS. We report two patients referred for OSAHS worsened by nasopharyngeal stenosis complicating UPPP. Both patients were treated with carbon dioxide laser release of adhesions and placement of a nasopharyngeal obturator. Follow-up polysomnogram… Show more

“…17 The first patient (a 31-year-old man with a BMI of 26.6 kg/m 2 ) showed a post-treatment decrease in AHI from 46 to 5.7 episodes per hour. 17 The second patient (a 32-year-old man with a BMI of 27.2 kg/m 2 ) showed a decrease in AHI from 57 to 4.7 episodes per hour. 17 Both patients had improvement in snoring and daytime somnolence.…”

“…According to Haavisto et al [22], hemorrhages can occur postoperatively in up to 14% of the patients. Stenoses in the nasopharyngeal region are described as complications to be feared after UPPP [23]. In our series, stenoses did not occur in any of the patients treated by means of our modified UPPP technique with preservation of the mucosal fold between uvula and the upper pole of the tonsils.…”

“…They considered that olfactory loss after UPPP seemed hard to explain by air flow changes only. Nasopharyngeal stenosis is a rare complication of UPPP [ 15 ]. It might change air flow through choanae and impair olfactory function, but nasopharyngeal stenosis was not observed in both of our cases by nasal endoscopy.…”
