Antibiotic resistance in bacteria has become a worrying phenomenon in today's world, K. pneumoniae is a member of the Enterobacteriaceae family, which causes nosocomial infections as an opportunistic pathogen but inherently harbored as a part of the natural human microbiota this study aims to inform the sensitivity of bacteria to different antibiotics commonly used in the treatment against K. pneumoniae strains isolated from different infection sites and hospital environmental samples. Samples were collected from different sources (n = 886) were recorded from Diwaniyah hospitals during the period from August 2019 to January 2020, Among the 275 isolates positive for bacterial culture, only 70 isolates (40%) were identified as K.pneumoniae by standard microbiological techniques and antibiotic susceptibility testing was done by Kirby-Bauer disc diffusion testing to 29 antibiotics using the tablet diffusion method according to CLSI 2020, The results susceptibility testing showed that frequency resistance of K. pneumoniae isolates to Pipercillin and Amoxicillin/ clavulanic acid was (100 %), Cefuroxime )97.14%) Cefoxitin (50%), Cefotaxime (74.28%), ceftriaxone (84.28%), aztreonam (52.85%), ceftazidime (81.42%), Tetracycline (70%), Nitrofurantoin (61.42%), Amikacin (57.14%), ciprofloxacin (50%). Also, the results showed the highest frequency of susceptibility among K.pneumoniae was associated withOfloxacin(78.57%), Netilmicin(74.28%), Gentamicin(75.71%), Meropenem(68.57%), Trimethoprim-Sulfamethoxazole(64.28%), Levofloxacin(72.85%).

In the last decades, antimicrobial resistance has become a global threat to public health systems worldwide. Among those bacteria that pose the greatest threat to human health because of its growing resistance to antibiotics are the members of the Enterobacteriaceae family, particularly Escherichia coli and Klebsiella spp. Among the different antibiotic-resistant mechanisms developed by bacteria, the ones found in Enterobacteriaceae are more diverse than those in other families and include resistance to different antibiotic groups, advantages that partially explain why these microorganisms are among the most common causes of antibiotic-resistant bacterial infections in humans. Due to the continuously increasing number of infections caused by multidrug-resistant E. coli due to its ease of transmission via the fecaloral route among humans and from environmental sources, the understanding of the epidemiology of these strains and their mechanisms of resistance are key components in the fight against these infections.

Beta-lactam antibiotics have the same mechanism of action, the inhibition of bacterial cell wall synthesis, mainly affecting peptidoglycan. They are considered safety antibiotics, since, when they directly reach the cell wall, they cause bacterial autolysis. The objective of this research is to evaluate the affinity between the beta-lactamase enzyme and several classes of beta-lactam antibiotics. This study is a research of exploratory type methodology, of quantitative nature with application of computer simulation with application of methods with computer simulation with the evaluation of the connection scores by traditional molecular anchoring using the AutoDock 4.2 program. We identified that, in the results of molecular docking simulations, penicillin and cephalosporin showed greater affinity at the catalytic site, due to a lower Gibbs free energy. It is concluded that the affinity of the antibiotics penicillin and cephalosporin may in the future represent an obstacle for the treatment of diseases such as, mild and moderate infections of the respiratory tract, skin infections, venereal infections, and the prophylaxis of rheumatic diseases. We suggest that further studies evaluate the simulations with molecular dynamics tests to see if, considering the flexibility of the enzyme, even more accurate results would be possible.
