Background: The aim of this study was to explore the nonlinear relationships between natural amenities and health at the intersection of sociodemographic characteristics among primary care patients with chronic conditions. Methods: We used survey data from 3409 adults across 119 US counties. PROMIS-29 mental and physical health summary scores were the primary outcomes. The natural environment (measured using the County USDA Natural Amenities Scale (NAS)) was the primary predictor. Piecewise spline regression models were used to explore the relationships between NAS and health at the intersection of sociodemographic factors. Results: We identified a nonlinear relationship between NAS and health. Low-income individuals had a negative association with health with each increase in NAS in high-amenity areas only. However, White individuals had a stronger association with health with each increase in NAS in low-amenity areas. Conclusions: In areas with low natural amenities, more amenities are associated with better physical and mental health, but only for advantaged populations. Meanwhile, for disadvantaged populations, an increase in amenities in high-amenity areas is associated with decreases in mental and physical health. Understanding how traditionally advantaged populations utilize the natural environment could provide insight into the mechanisms driving these disparities.

Relación entre la exposición a pesticidas y las enfermedades mentales: Una revisión sistemática

Este trabajo realiza una revisión sistemática sobre la relación existente entre la exposición a residuos de pesticidas y la presencia de enfermedades o trastornos mentales. Los pesticidas son sustancias químicas utilizadas en la inmensa mayoría de los campos agrícolas para prevenir o eliminar las plagas que amenazan a los cultivos. Sin embargo, su uso puede implicar serios problemas para la salud de las personas como enfermedades pulmonares, cáncer, enfermedades metabólicas, problemas de infertilidad e incluso trastornos psicopatológicos. La metodología para realizar este estudio de revisión siguió las indicaciones marcadas por el protocolo PRISMA, encontrándose un total de cuatro artículos originales tras aplicar los criterios de exclusión e inclusión establecidos. Tres artículos estudiaron la relación exposición a residuos pesticidas - enfermedades mentales en sujetos con características diferentes y con niveles de exposición variables. Única-mente un artículo empleó ratones sometidos a diferentes dosis de pesticidas como modelo para evaluar su efecto sobre el comportamiento de estos animales. De los resultados encontrados por los diferentes artículos se puede deducir que la exposición a residuos de pesticidas tiene claros efectos sobre la salud mental de individuos altamente susceptibles a la presencia de contaminantes como mujeres lactantes y recién nacidos; así como personas sometidas a elevados niveles de pesticidas como formuladores y aplicadores agrícolas o personas residentes en las cercanías de campos agrícolas. Por otro lado, futuras investigaciones sobre este tema, basadas en modelos animales y estudios poblacionales, son necesarias para establecer conexiones realistas entre los trastornos mentales y los diferentes tipos de pesticidas usados actualmente en los campos agrícolas.

The sustainable success of water, sanitation, and hygiene (WASH) interventions to reduce waterborne disease and promote public health in low‐income regions is uncertain. Recent large randomized controlled trials in Bangladesh, Kenya, and Zimbabwe revealed that traditional household WASH interventions are unlikely to reduce diarrhea or improve linear growth (i.e., reduce stunting) in infants and children—two important indicators of the effectiveness of WASH interventions. A possible factor contributing to the null results is that traditional WASH interventions control microbial pathogens but do not remove chemical toxicants. Exposure to many organic and inorganic chemical toxicants is associated with diarrheal symptoms, as well as with preterm birth and stunting. Many chemical toxicants have deleterious effects on the immune system, impairing the body's response to infectious diseases, and reducing the effectiveness of vaccinations. Globalization has led to rapid and largely unregulated import, production, use, and disposal without adequate treatment of large quantities of hazardous chemicals throughout the developing world, with profound effects on the environment and human health. Literature evidence from the past 10 to 15 years on putative and demonstrated chemical exposures in Bangladesh, Kenya, and Zimbabwe and the regions within the countries where the large WASH trials took place supports potential contribution(s) from chemical toxicants in the failure of traditional WASH interventions to relieve diarrhea and stunting. Mitigating exposures to organic and inorganic chemical immunotoxins through drinking water and other routes is likely to be a necessary‐but‐insufficient criterion for the sustainable success of WASH interventions.
