Resection of segment VIII for hepatocellular carcinoma

Abstract: Complete resection of SVIII or its subsegments can be performed safely and the long-term outcomes seem acceptable. This can be a curative procedure for HCC, especially in patients with limited liver function reserve, in whom right hepatectomy or right paramedian sectorectomy might otherwise be needed.

“…Intraoperative ultrasonography was an essential procedure for the identification of hepatic tumors and intrahepatic vessels. A dye-staining technique (injection of indigo carmine and/or indocyanine green into the portal veins) [11-13] was used to delineate segmental boundaries on the liver surface, if needed. The hepatic parenchyma was divided using a clamp-crushing technique under intermittent hepatic inflow occlusion.…”

“…Between April 2000 and January 2013, 645 patients underwent liver resection for HCC at our institution. Among 509 patients undergoing initial liver resection, 255 had a single HCC 5 cm or less in diameter, and 131 of them had liver damage A [11,13] . After excluding 26 patients who underwent segmentectomy or hemihepatectomy, the final remaining total of 105 patients who underwent subsegmentectomy were evaluated ( Fig.…”

“…In those reports, however, the AR was segmentectomy and/or hemihepatectomy, and few previous studies have compared LR with AR (subsegmentectomy) in patients with different types of liver damage [10,11] . The purpose of the present historical cohort study was to determine the outcome of AR or LR in patients with NLF and a single lesion HCC, as well as to identify predictors of HCC recurrence.…”
