Resection of Metachronous Adrenal Metastasis after Liver Resection and Transplantation for Hepatocellular Carcinoma

Abstract: Background: This study analyzed the patient survival outcomes following the resection of hepatocellular carcinoma (HCC) metachronous adrenal metastasis (MAM) in patients who had undergone liver resection or liver transplantation (LT). Methods: Clinical results were analyzed retrospectively in 26 patients with MAM-HCC who underwent adrenalectomy. Results: The mean interval between initial surgery and adrenalectomy was significantly shorter in the resection group than in the LT group (18.3 ± 14.4 vs. 42.6 ± 13.8… Show more

“…We previously reported a beneficial effect of surgical metastasectomy of metachronous pulmonary and adrenal metastases from HCCs on patient survival. 12 13 There are only a small number of studies supporting resection of tumors arising from peritoneal seeding of HCC, and resection of peritoneal metastases should only be considered in patients whose primary liver neoplasm is under control and who have no metastases in other organs. 14 15 16 Since there was no evidence of tumor recurrence other than localized peritoneal metastasis in the two cases described here, we decided to perform peritoneal metastasectomy in both patients.…”

“…About 10-23% of patients with recurrence survive for a long time after surgical therapy (6,8,32). The median survival of unresectable cases was reported to be 5-15 months, while that of resectable cases with local recurrence was reported to be 28-65 months (33)(34)(35); cases of long-term survival after resection of the grafted liver, lung, and adrenal metastasis have also been reported (33,34,(36)(37)(38).…”

“…Left adrenal HCC metastases after LT are extremely rare [1,2] and reported mainly in UCSF and Milan-OUT LT recipients [3,4] (Table 1). To our knowledge, this is the first case reported of adrenal HCC metastasis that occurred after 2 years on the left adrenal gland in a Milan-IN patient at transplant.…”
