Highlights BackgroundThe Africa Middle East (AfME) Cardiovascular Epidemiological (ACE) study demonstrated that cardiovascular risk factors are highly prevalent among relatively young adult outpatients attending general practice clinics across AfME regions. ObjectiveBased on broader AfME estimates from the ACE Study, this sub-analysis evaluated the prevalence of cardiovascular risk factors in the United Arab Emirates (UAE), particularly in rural and urban cohorts attending general practice clinics. MethodsData from the cross-sectional ACE study were used, comprising stable, adult outpatients attending general practice clinics in the UAE. The prevalence of six cardiovascular risk factors was analyzed: hypertension, diabetes, dyslipidemia, obesity, smoking, abdominal obesity. Rural populations were defined as living >50km from urban centers, or lack of transportation. ResultsIn this cohort of 495 patients (aged 45.1 years; 49.8% female) from the UAE, a high prevalence of abdominal obesity (71.5%) and dyslipidemia (74.0%) was found. Nearly half of patients had hypertension (43.0%) and one-third diabetes (32.4%). Nearly all outpatients (92.9%) had ≥1 modifiable risk factor (74.9% had ≥1, 59.7% had ≥3). Observations were similar by gender, and across urban and rural centers. Many outpatients with pre-existing hypertension or dyslipidemia did not meet recognized blood pressure or low-density lipoprotein cholesterol goals. ConclusionsCardiovascular risk factors are prevalent among relatively young adult, clinically stable outpatients attending clinics across the UAE. These findings support targeted screening of outpatients visiting a general practitioner, which may provide opportunity for early discovery and ongoing management of risk factors, including recommending lifestyle changes. The ACE trial is registered under NCT01243138.

The connection concerning the autonomic nervous system and coronary spasm during the systole is multifaceted. An augmentation in sympathetic activity, due to pain caused by angina and the several transient ischemic episodes may origin an augment in noradrenaline, the neurotransmitter of efferent sympathetic fibers, triggering more vasoconstriction by stimulating vascular smooth muscle cells, besides may lead the cardiac hypertrophy. Based on these concepts we aim to find a role to renal sympathetic denervation in patients with symptomatic myocardial bridging refractory to standard clinical treatment and ventricular arrhythmias. In conclusion, our findings suggest that RSD can be a role in myocardial bridging treatment, augmenting the LVEF, diminishing the LV mass and the number of transient ischemic segments measured by CMRI, besides to reduce the number of individuals presenting symptoms, the mean of NSVT recorded by 24-hour-Holter monitoring, and the number of patients with SVT inducible by the EPS. Perhaps such benefits are due to the decrease in the LV mass and sympathetic cardiac activity, consequently, there being less constriction of the arteries with myocardial bridges and less ventricular arrhythmias.We report preliminary data on 6 patients with controlled hypertension, with normal renal function, with symptomatic myocardial bridging refractory to clinical treatment and ventricular arrhythmias who underwent a pilot renal sympathetic denervation (RSD) procedure. At baseline, the 6 (100%) patients presented symptoms while 6 months after RSD only 1 (17%) subject still complained of the symptoms (P=0.0152). Our findings suggest that RSD can play a role in myocardial bridging treatment, augmenting LVEF, diminishing LV mass and the number of transient ischemic segments measured by CMRI.

Estimation of Risk Factors for Cardiogenic Shock in Takotsubo Cardiomyopathy: A Retrospective Study

Introduction: Presentation of Takotsubo cardiomyopathy(TCM) widely varies amongst the patients, ranging from asymptomatic patients to cardiogenic shock or even cardiac arrest. The underlying risk factors inherent to the patient that predispose to a catastrophic presentation have not been delineated well in the past.Objective: To distinguish risk factors and presenting characteristics of patients diagnosed with cardiogenic shock from TCM.Methods: Total 51 angiography-confirmed TCM patients admitted in Upstate Hospital from 2010 – 2014 were compared retrospectively, 13 presenting with cardiogenic shock.Results: While TCM was predominantly common in elderly females, four of the total six male TCM patients presented with shock (Odds ratio= 8.0, p=0.027). TSH was higher in cardiogenic shock group. Patients with shock had significantly higher incidence of ST elevations on EKG and higher peak troponins. Moreover, ST elevations were predominantly in the inferior, anteroseptal and lateral leads in the shock group as compared to only the septal leads in the control group. Echocardiography revealed lower LV ejection fraction, LV outflow tract obstruction and concomitant significant mitral regurgitation in shock patients. No correlation was found with age, BMI, coronary disease risk factors (diabetes, hypertension, smoking), coincident infection, electrolyte imbalance, QTc interval length.Conclusion: Risk factors and pathophysiology for cardiogenic shock in TCM were identified. TCM in males is rare, but male gender is a risk factor for shock.
