Reproductive outcome in 326 women with unicornuate uterus

Abstract: We found that a unicornuate uterus is associated with a lower live-birth rate and a higher risk of ectopic pregnancy, miscarriage, preterm delivery and malpresentation at birth in comparison to pregnancies in which the uterus is normally formed. Women with a unicornuate uterus were more likely to have adenomyosis and endometriosis, the risk of the latter being greatest in those with a functional rudimentary horn. What are the clinical implications of this work? These findings should inform the counseling of wom… Show more

“…Studies also indicate a higher risk of developing pelvic endometriosis in women with a unicornuate uterus with a residual horn compared to women with a uterus without a functional horn. In the given research groups, there were no differences in the incidence of adenomyosis, although it occurs much more often in patients with a unicornuate uterus compared to women with a properly shaped uterus [12].…”

“…The procedure reduces the risk of an ectopic pregnancy developing first in the residual horn, which can lead to rupture and life-threatening hemorrhage. A reduced risk of developing adenomyosis after the procedure [12] has also been proven.…”

“…Existing literature suggests that pregnancy outcomes for women with a unicornuate uterus are relatively unfavorable compared to those with a normal uterus [ 8 ]. As far as we know, this is the first case of pregnancy through IVF following treatment of NCRHP.…”

“…Pregnancies in women with a unicornuate uterus are recognized as high-risk due to an increased likelihood of preterm births. A recent study comparing women with a unicornuate uterus to a control group matched for BMI and age found increased rates of both miscarriage and preterm delivery, with an adjusted odds (aOR) ratio of 3.04 [ 8 ]. The underlying factor for these adverse outcomes is believed to be the relatively limited size of the unicornuate uterus, leading to insufficient expansion during pregnancy.…”

“…Comparing our case to similar case reports, several common challenges and risks associated with unicornuate pregnancies emerge. Fetal growth restriction and preterm labor are frequently encountered in pregnancies with a unicornuate uterus [6][7][8]. These risks are attributed to the limited uterine expansion capacity and the developing fetus' altered blood supply.…”
