Reproductive history in women with abdominal aortic aneurysms

Abstract

These findings suggest that women with larger AAA reach menopausal age earlier, and this could influence an earlier onset of aneurysmatic disease or an increase in aneurysm growth. The true role of endogenous estrogen in aneurysm development and expansion is yet to be investigated.

“…Results demonstrated smoking as a risk factor for AAA while diabetes was protective, but an interesting aspect of this study was the segregation of women who had an AAA ≥5 cm versus <5 cm that showed diferences in onset of menopause. Women who had large AAAs were approximately 2 years younger at age for menopause than those women who had smaller AAAs [25]. These data suggest that ovarian hormones may play a role in protection from large AAA development.…”

“…Данные о гендерных различиях патогенеза, клинико-анатомической картины, характера послеоперационных осложнений как при открытых, так и при эндоваскулярных вмешательствах фрагментарны и во многом противоречивы. В то же время известно, что у женщин, в сравнении с мужчинами, риск разрыва АИСА в 3-4 раза выше, средний диаметр аневризмы аорты на момент разрыва меньше [2,4,7,[9][10][11][12][13][14][15][16][17][18], а результаты планового и экстренного хирургического лечения хуже [4,5,9,[11][12][13][15][16][17][18][19][20][21][22]. Так, в одном из исследований, включавшем в себя более 23 000 больных с АИСА, установлены статистически значимые гендерные различия уровня госпитальной летальности при плановых открытых и эндоваскулярных операциях (у женщин -6,9 и 1,8 %, а у мужчин -4,0 и 0,7 % соответственно).…”
