Reproducibility and Validity of Lung Density Measures from Cardiac CT Scans—The Multi-Ethnic Study of Atherosclerosis (MESA) Lung Study1

Abstract: Purpose Cardiac CT scans for the assessment of coronary calcium scores include approximately 70% of the lung volume and may be useful for the quantitative assessment of emphysema. The reproducibility of lung density measures from cardiac CTs and their validity compared to lung density measures from full-lung scans is unknown. Methods and Methods The Multi-Ethnic Study of Atherosclerosis (MESA) performed paired cardiac CT scans for 6,814 participants at baseline and at follow-up. The MESA-Lung Study assessed … Show more

“…Previous studies that have reported inter-and intra-observer repeatability in a smoking population (19, 20) have used either spirometric or effort-driven volume control, but have rarely accounted for volume deviations. We utilized a volume-controlled pneumo-tachometer with volume monitoring that allowed us to carefully account for volume differences between scan.…”

“…12 The upper lobes are classically involved in smoking-related centrilobular emphysema, and a recent study showed that upper-lobepredominant emphysema was signifi cantly associated with greater lung function decline in former and current heavy smokers. 28 However, cardiac scans assess lower and central regions of the lung, which receive the greatest amount of pulmonary blood fl ow in the upright position.…”

“…We assessed emphysema quantitatively, which has much greater precision than radiologist assessment but for which a standard threshold of normality is currently lacking. 58,59 Therefore, we used an arbitrary but reasonable threshold of the median value of percent emphysema. Sensitivity anal yses showed that results were similar using a 25th percentile threshold and using a 2 950 HU definition (median or 25th percentile) to defi ne percent emphysema.…”
