Ö ÖZ ZE ET T A Am ma aç ç: : Rebound tonometre cihazı ile yumuşak kontakt lens uygulanmadan önceki ve uygulandıktan sonraki göz içi basıncı (GİB) değerlerini karşılaştırmak ve bu değerlere kontakt lens materyalinin etkisini araştırmaktır. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Kontakt lens polikliniğimize başvuran 87 hastanın 87 gözü çalışmaya alındı. Hastaların 43'üne Lotrafilcon A (Airoptix, Cibavision), 44'üne Balafilcon A (Purevision HD, Bausch&Lomb) kullanıldı. Kontakt lens uygulanmadan önce ve uygulandıktan bir saat sonra iCare tonometre (Tiolat OY, Helsinki, Finlandiya) ile GİB ölçümü yapıldı. B Bu ul lg gu ul la ar r: : Uygulanan kontakt lenslerin ortalama kırıcılık değeri Lotrafilcon grubunda -0,91±2,79 dioptri (D) (-5D ile 5D arasında) ve Balafilcon grubunda -1,13±2,94 D (-5D ile 5D arasında) idi. Kontakt lens uygulanmadan önceki ortalama GİB 17,7±3,4 (8,1-25) mmHg, kontakt lens uygulandıktan sonraki ortalama GİB 18,5±3,6 (8,1-28) mmHg idi (p=0,004). Lotrafilcon grubunda kontakt lens uygulanmadan önceki ortalama GİB 17,0±3,0 (8,1-24) mmHg ve kontakt lens uygulandıktan sonraki ortalama GİB 17,4±3,3 (8,1-27) mmHg idi (p=0,333). Balafilcon grubunda kontakt lens uygulanmadan önceki ortalama GİB 18,3±3,8 (8.9-25) mmHg ve kontakt lens uygulandıktan sonraki ortalama GİB 19,6±3,7 (8,9-28,5) mmHg idi (p=0,003). Kontakt lens diyoptrisinin, GİB değişimine bir etkisi olmadığı görüldü (Pearson korelasyon analizi r=0,028, p=0,800). Tüm hastaların 26 (%29,9)'sı hipermetropik, 61 (%70,1)'i miyopik kırma kusuruna sahip olup, kontakt lens uygulandıktan sonra, hipermetropik gruptaki GİB değişiminin istatistiksel açıdan anlamlı farklılık göster-diği görüldü (Hipermetrop grupta p=0,004, miyop grupta p=0,109). Tonometry is fundamental to routine ophthalmological evaluation, and Goldmann applanation tonometry (GAT) is the gold standard method. Several devices, including the Perkins tonometer, Tono-Pen, Pascal tonometer (dynamic contour), and non-contact tonometer, can provide reliable values in adults (1)(2)(3)(4) . However, in general practice, intraocular pressure (IOP) measurement can be difficult in children due to a lack of cooperation. ABSTRACTPurpose: High intraocular pressure (IOP) is an important risk factor for a variety of pediatric ophthalmic conditions. The purpose of this study is to evaluate the feasibility, length of examination, and corneal epithelial damage induced by rebound tonometry (RBT) versus Goldmann applanation tonometry (GAT) in school children. Methods: Healthy children (n=57) participated in a randomized, transversal study with IOP measurement by GAT followed by RBT (study arm 1) or RBT followed by GAT (study arm 2). The number of attempts to acquire a reliable IOP measurement and the length of the examination were quantified. Corneal epithelial damage induced by tonometry was evaluated. Bland-Altman analysis was performed to establish the level of agreement between the two techniques. Results: The IOP was measured in all children with at least one of the devices. In both study arms, more children failed to be examined with GAT than with RBT (26% vs. 4%, and 16% vs. 6%, p<0.001, in study arm 1 and 2, respectively). The length of examination was shorter for RBT than for GAT (67.81 s ± 35.20 s vs. 126.70 s ± 56.60 s; p<0.0001); IOP measurements with RBT in both study arms were higher than those with GAT (15.20 ± 2.74 mmHg vs. 13.25 ± 2.47 mmHg, p=0.0247 and 16.76 ± 3.99 mmHg vs. 13.92 ± 2.08 mmHg, p=0.003, respectively). No difference was observed between RBT and GAT regarding the corneal epithelial damage caused by tonometry. Conclusion: IOP measurement is feasible in a greater number of children with RBT, and the examination was faster than that for GAT. Compared with GAT, RBT tended to overestimate the IOP. None of the methods induced marked corneal epithelial defects.

This review discusses the results of various studies conducted in recent years on the comparison of modern methods of measuring intraocular pressure (IOP) in children: pneumotonometry, Maklakov applanation tonometry, and tonometry using Perkins tonometer, Goldmann tonometer, Icare tonometer, Ocular Response Analyzer, TonoPen handheld tonometer, transpalpebral tonometer TIOP01, or a dynamic contour Pascal tonometer. This study discusses the advantages and disadvantages of different methods of measurement of IOP in children, including the evaluation of patients with fibrous lens capsules that might affect the measurement of IOP and an analysis of the characteristics of evaluation of IOP in children with congenital glaucoma.

Abstract: Measurement of IOP with the RBT is a highly reproducible method in schoolchildren showing high intraobserver and interobserver correlation and it seems to be very comfortable when performing IOP measurements in schoolchildren without an anesthetic.
