Reporting of outcomes in randomized controlled trials on nail psoriasis: a systematic review

Abstract: Heterogeneity in the type and reporting of nail psoriasis outcome instruments needs to be addressed in the process towards COS development. Sufficient reporting of instrument characteristics should be encouraged. As nail psoriasis is generally assessed secondarily to psoriasis of the skin or joints, collaboration between different research groups in COS development is needed.

“…The findings of our literature search match that of a systematic review conducted in 2015 (published in 2018, [ 30 ]) that aimed to identify both clinical and patient-reported NPso outcome measures used in RCTs. They found that HRQoL was assessed in 43% of the included studies, mainly using the DLQI, but also the NPQ10 and the NAPPA-QOL.…”

“…Even though the NAPSI has even been used in clinical trials to quantify the treatment effect 10 , 11 , the score is not used in clinical practice. The greatest constraint of the NAPSI is the time necessary for the grading of a single patient 12 .…”

“…As a result, multiple instruments have been developed to assess nail psoriasis. The most widely utilized system for measuring nail psoriasis in clinical trials is the Nail Psoriasis Severity Index (NAPSI) [ 15 ]. To calculate the NAPSI score, each nail is divided into quadrants and scored for the absence or the presence (0 or 1) of psoriasis manifestations in the nail bed and matrix [ 15 ].…”

To calculate the NAPSI score, each nail is divided into quadrants and scored for the absence or the presence (0 or 1) of psoriasis manifestations in the nail bed and matrix [ 15 ]. Scores are summed to obtain total NAPSI nail score with a possible range of 0–160 (where a higher score indicates greater severity of nail psoriasis).…”
