Report of the second dementia with Lewy body international workshop

Abstract: Clinical management of DLB patients usually centers on the treatment of noncognitive features. There is now a pressing need to establish appropriately designed randomized controlled trials in DLB. Collaboration between dementia and movement disorder specialists is essential for rapid progress in research and clinical management protocols.

“…The dominating protein component normally behaves as a monomeric protein, and factors causing the transition from monomeric to fibrillar states are under intense investigation. Lewy bodies and Lewy neurites, the pathological hallmarks of Parkinson's disease and dementia with Lewy bodies (2), are intraneuronal inclusions comprising ␣-synuclein-containing filaments as the major component (3,4). Mutations in the ␣-synuclein gene may elicit rare forms of early-onset Parkinson's disease (5,6).…”

“…Dementia with Lewy bodies is the second most common form of dementia next to Alzheimer's disease (2). Hence, the pathognomonic cortical Lewy bodies are common neuronal lesions, but they have been less intensely studied as compared with brainstem Lewy bodies.…”

“…18 More specifically: for an AD diagnosis, the patient must have no or mild WMC, and predominant parietotemporal lobe symptoms. Frontotemporal dementia is diagnosed according to Neary et al; 19 Lewy-body dementia according to McKeith et al; 20 and primary progressive aphasia (PPA) according to Gorno-Tempini et al 21 Vascular dementia forms are either SVD or cortical vascular dementia (cVaD). Subcortical vascular dementia is diagnosed using the Erkinjuntti criteria (2000), 22 and cVaD using the National Institute of Neurological Disorders and Stroke and Association Internationale pour la Recherche´et l'Enseignement en Neurosciences criteria.…”

“…The final CDR rating was based on the pattern of box scores (Morris, 1993). Dementia subtyping was accomplished after the conference by the study neurologist through chart review using established criteria for probable0possible AD (McKhann et al, 1984), probable0possible vascular dementia (VaD; Chui et al, 1992), probable0possible Lewy body dementia (McKeith et al, 1999), and frontotemporal dementia (Knopman et al, 2005).…”
