Lyme disease is an infection caused by bacteria known as Borrelia burgdorferi sensu lato, is the most widespread vector-borne disease in the United States, and transmitted to human beings through the bite of Ixodes tick. However, significant portion (10-30%) individuals may experience ancillary nonspecific symptoms that last months after the completion of therapy. The group of symptoms known as post-treatment Lyme disease syndrome (PTLDS), a form of the more general term "chronic Lyme disease," includes tiredness, cognitive impairment, and musculoskeletal discomfort. These symptoms are linked to disability and last longer than six months. Patients with nonspecific symptoms assumed to be caused by a supposed persistent Borrelia burgdorferi infection and may or may not exhibit signs of Lyme disease are referred to as having chronic Lyme disease. The diagnosis of PTLDS and chronic Lyme disease has become more and more important in immunologists' practices because it is difficult to diagnose. ELISA and western blot testing are useful in diagnosing Lyme disease. The cornerstone of care is still antibiotic therapy. This chapter reviews the evidence that supports current understanding of the life cycle, historical biogeography, and evolution of Ixodes spp., the ticks that carry the Lyme disease, as well as their methods of dispersal, and disease-transmission mechanisms, and the efficacy of vector control interventions.

Резюме. Лайм-бореліоз (ЛБ) – кліщова інфекційна хвороба з мультисистемними проявами, спричинена геновидами комплексу Borrelia burgdorferi s. l. В Україні з 2000 до 2020 р. зареєстровано більше 30 000 захворювань. ЛБ перебігає в двох формах: еритемній (мігруюча еритема) та безеритемній (БЕФ) (ураження опорно-рухової, нервової, серцево-судинної систем, очей). Мета дослідження – удосконалити комплексне лікування хворих на Лайм-бореліоз, у тому числі за наявності в них супутнього гранулоцитарного анаплазмозу людини (ГАЛ), за допомогою поєднаного застосування цефтріаксону та доксицикліну гідрохлориду. Матеріали і методи. Під спостереженням перебувало 45 хворих на Лайм-бореліоз віком від 22 до 77 років, яких лікували амбулаторно та стаціонарно в КНП «Тернопільська обласна клінічна лікарня» та інфекційному відділенні КНП «Тернопільська міська клінічна лікарня швидкої допомоги». У 23 (51,1 %) обстежених, окрім Лайм-бореліозу, діагностовано гранулоцитарний анаплазмоз людини. Апробували 2 схеми лікування Лайм-бореліозу: цефтріаксон протягом 28 днів (схема І) та цефтріаксон протягом 14 днів із подальшим призначенням доксицикліну гідрохлориду протягом 14 днів (схема ІІ). Ефективність лікування оцінювали за активністю ураження суглобів, використовуючи індекс DAS 28, строками зникнення клінічних проявів та динамікою прозапальних цитокінів TNFα, ІL6 і протизапального ІL4 у сироватці крові пацієнтів. Результати. Поєднання цефтріаксону з доксицикліном гідрохлоридом дозволило збільшити частку осіб із низьким ступенем активності запалення суглобів у 10 разів, порівняно з початком лікування, і в 2 рази щодо 14-го дня терапії лише цефтріаксоном, зменшити відсоток осіб із високим ступенем активності процесу в 8 разів, порівняно з числом пацієнтів із даним ступенем активності на початку лікування, і в 4 рази, порівняно з аналогічною групою обстежених на 14-й день лікування лише цефтріаксоном. Застосування цієї схеми лікування хворим із БЕФ ЛБ+ГАЛ дало змогу забезпечити зникнення гарячки, лімфаденопатії й втоми/загальної слабкості у пацієнтів і досягти зменшення відсотка пацієнтів з болем голови в 12 разів щодо початку лікування і в 7 разів порівняно з даними на 14-й день терапії цефтріаксоном. Лікування цефтріаксоном і доксицикліном гідрохлоридом за випробуваною схемою для хворих із поєднаними кліщовими інфекціями (ЛБ і ГАЛ) дозволило статистично достовірно знизити концентрацію прозапальних цитокінів TNFα та IL6 у крові в 2 та 3 рази відповідно. Висновки. Для лікування хворих на Лайм-бореліоз упоєднанні з гранулоцитарним анаплазмозом людини рекомендовано застосовати поступово два антибактерійні препарати «Цефтріаксон» і «Доксицикліну гідрохлорид».

Three patients with relapsing and remitting borreliosis, babesiosis, and bartonellosis, despite extended anti-infective therapy, were prescribed double-dose dapsone combination therapy (DDDCT) for 8 weeks, followed by one or several two-week courses of pulsed high-dose dapsone combination therapy (HDDCT). We discuss these patients’ cases to illustrate three important variables required for long-term remission. First, diagnosing and treating active co-infections, including Babesia and Bartonella were important. Babesia required rotations of multiple anti-malarial drug combinations and herbal therapies, and Bartonella required one or several 6-day HDDCT pulses to achieve clinical remission. Second, all prior oral, intramuscular (IM), and/or intravenous (IV) antibiotics used for chronic Lyme disease (CLD)/post-treatment Lyme disease syndrome (PTLDS), irrespective of the length of administration, were inferior in efficacy to short-term pulsed biofilm/persister drug combination therapy i.e., dapsone, rifampin, methylene blue, and pyrazinamide, which improved resistant fatigue, pain, headaches, insomnia, and neuropsychiatric symptoms. Lastly, addressing multiple factors on the 16-point multiple systemic infectious disease syndrome (MSIDS) model was important in achieving remission. In conclusion, DDDCT with one or several 6–7-day pulses of HDDCT, while addressing abnormalities on the 16-point MSIDS map, could represent a novel effective clinical and anti-infective strategy in CLD/PTLDS and associated co-infections including Bartonella.
