BACKGROUND In the quest to manage hepatocellular carcinoma (HCC), the focus has shifted to a more holistic approach encompassing both data analytics and innovative treatments. Analyzing rich data resources, such as the cancer genome atlas (TCGA), and examining progressive therapies can potentially reshape the trajectory of HCC treatment. AIM To elucidate the immunological genes and the underlying mechanism of the combined Kombo knife and sorafenib regimen for HCC by analyzing data from TCGA and machine learning data. METHODS Immune attributes were evaluated via TCGA's postablation HCC RNA sequencing data. Using weighted gene coexpression network analysis and machine learning, we identified genes with high prognostic value. The therapeutic landscape and safety metrics of the integrated treatment were critically evaluated across cellular and animal models. RESULTS Immune genes–specifically, peptidylprolyl isomerase A and solute carrier family 29 member 3–emerged as significant prognostic markers. Enhanced therapeutic outcomes, such as prolonged progression-free survival and an elevated overall response rate, characterize the combined approach, with peripheral blood mononuclear cells displaying potent effects on HCC dynamics. CONCLUSION The combination of Kombo knife with sorafenib is an innovative HCC treatment modality anchored in immune-centric strategies.

This paper aims to analyze and evaluate the performance of different classification algorithms on medical datasets and gives some tips and guidelines for choosing the right algorithm for the dataset. For this purpose, twelve medical datasets were selected from the UCI repository, including breast cancer data, chronic kidney disease, cryotherapy, hepatitis, immunotherapy, Indian Liver Patient Dataset (ILPD), liver disorders, pima diabetes, risk factors cervical cancer, statlog (heart) dataset, dermatology and lung cancer. The selected classifiers for performance evaluation are Bagging, IBK, J48, JRip, Multilayer Perceptron (MP), Naïve Bayes (NB), Support Vector Machine (SVM), ZeroR and Voting Frequency Intervals (VFI). The algorithms were implemented on datasets using the WEKA tool and 10-fold cross-validation. Experimental results and analysis for all measures were introduced to guideline researchers who seek to achieve and how to balance different measures. Additionally, challenges and future directions were presented at the end of this paper.

Hepatocellular carcinoma (HCC), a challenging malignancy, often necessitates surgical intervention, notably liver resection. However, the high recurrence rate, reaching 70% within 5 years post-resection, significantly impacts patient outcomes. Neoadjuvant therapies aim to preoperatively address this challenge, reducing lesion size, improving surgical resection rates, deactivating potential micrometastases, and ultimately lowering postoperative recurrence rates. This review concentrates on advances in research on and clinical use of neoadjuvant therapies for HCC, with particular attention to the use of immune checkpoint inhibitors (ICIs) targeting programmed cell death-1 (PD-1), programmed cell death ligand-1 (PD-L1), and cytotoxic T-lymphocyte-associated protein-4 (CTLA-4). Ongoing clinical studies exploring immunotherapy combined with a tyrosine kinase inhibitor (TKI), interventional therapy, radiotherapy, and other modalities offer promising insights into overcoming resistance to monotherapies. In summary, neoadjuvant therapies hold significant promise in terms of improving the prognosis for patients with HCC and enhancing long-term survival, particularly through innovative combination strategies.
