Reply: Efficacy of Metformin and Topiramate in Prevention and Treatment of Second-Generation Antipsychotic–Induced Weight Gain

If this study is removed, it would appear that all of the studies reviewed would support topiramate's use for attenuation and prevention of antipsychotic-induced weight gain. Indeed, we would argue that the body of clinical evidence is more consistently positive for topiramate than for metformin. At the same time, we agree with Ellinger and colleagues that the safety profile of metformin appears to be more favorable compared to that of topiramate. Finally, we also advocate for larger, controlled studies to assess the beneficial effects of topiramate and metformin on the prevention and attenuation of antipsychotic-induced metabolic aberrations.
