Replacement of heart valves with autologous fascia lata. Surgical technique

Abstract: Autologous fascia lata graft valves attached on to a Dacron‐covered titanium frame have been used since April, 1969, at the General Infirmary at Leeds and the National Heart Hospital and Guy's Hospital, London. Seventy‐seven such grafts have been inserted into 70 patients with very good immediate and short‐term clinical and haemodynamic results. No systemic emboli have occurred although anticoagulants have not been used. The technique of preparing supported autologous fascia lata grafts for clinical use is out… Show more

“…During the same procedure, he constructed the valve during the operation by mounting the living fascia on a Dacron-covered titanium frame in the shape of a threecusp valve [29]. He began the clinical implantation of these valves in April 1969 [31,32]. This procedure of valve construction and implantation was used in many centers worldwide for about 3 years.…”

“…All patients had radiographic evidence of cardiomegaly, with left atrial enlargement and pulmonary venous congestion. The majority had a long history of slow but progressive deterioration and were accepted for operation with the knowledge that they formed a high risk group. The surgical technique has previously been described in detail (Ionescu et al, 1970). The valves were prepared at the time of operation and mounted on a metal frame in 45 and a polypropylene support in 3.…”

“…56 TYPE OF VALVE GRAFTS USED Three basically similar types of tissue grafts were used in this series. The three cusp valve grafts were made of biological tissue attached to Dacron-covered titanium frames' (lonescu et al, 1970b;Ionescu et al, 1972;Bartek, Holden, and Ionescu, 1974). Valves prepared from the patient's own fascia lata at the time of surgery, under sterile conditions, were used in 80 cases (25 in the aortic, 52 in the mitral, and 3 in the tricuspid position).…”
