Purpose: To determine the long-term repeatability of optical coherence tomography angiography (OCTA) parameters in normal eyes. Methods: We prospectively enrolled 104 normal eyes. Participants were divided into three groups based on differences in the signal strength (SS) of OCTA scans obtained at two visits at least 6 months apart: group 1, SS difference = 2; group 2, 1; group 3, 0. We measured a foveal centred scan area of 3 3 3 mm pattern. All measurements were performed twice at 5-min intervals at the initial visit. The second measurements were performed at least 6 months later. Results: In short-term vessel density (VD) repeatability, the coefficient of variation (CV) ranged from 3.89% to 8.10% and the intraclass correlation coefficient (ICC) ranged from 0.785 to 0.941. In terms of long-term repeatability, the CV was 5.39-12.62% and the ICC was 0.598-0.867. In group 1, the ICCs of the inner and full VDs were 0.194 and 0.221, respectively. In groups 2 and 3, the ICCs of the inner and full VDs were 0.611 and 0.603, and 0.763 and 0.765, respectively. The central VDs of all groups had high CV besides high ICC.Conclusions: Optical coherence tomography angiography parameters showed good short-term and reasonable long-term repeatability in normal individuals. The differences in SSs between measurements affected such repeatability, so physicians should query the reliability of OCTA parameters when the differences in SS are 2 or more even when the SS is high.

The prevalence of myopia is as high as 80% in Asia and 25% in other regions [1][2][3]. Severe myopia can increase the risk of glaucoma, retinal detachment, chorioretinal atrophy, and lacquer cracks. Therefore, high myopia patients, even those without other ophthalmic diseases, should not be overlooked [4]. In myopia, the sclera and chorioretina are enlarged to reduce the thickness of the macular choroid, which may result in delayed choroidal circulation to induce an ischemic state of Purpose: To analyze the repeatability of manual measurement of foveal avascular zone (FAZ) area in an optical coherence tomography angiography (OCTA) image in high myopia. Methods: This study comprised patients with high myopia and controls. Two consecutive FAZ areas of the superficial and deep capillary plexus were obtained using OCTA. The intraclass correlation coefficient (ICC) and coefficient of variation (CV) were assessed, and univariate and multivariate generalized linear mixed models were conducted to identify factors related to repeatability. Results: Thirty eyes with high myopia and 34 eyes of healthy subjects were included in the study. The mean age in high myopia and control subjects was 55.5 and 60.8 years, respectively, the mean spherical equivalent was -9.98 and -0.55 diopters, and the mean axial length was 28.0 and 23.9 mm. The ICCs of FAZ area of the superficial capillary plexus (SCP) were 0.891 and 0.919, while the CVs were 8.8% and 8.5%. In measurement of the deep capillary plexus, the ICCs were 0.788 and 0.907, while the CVs were 11.2% and 11.0%, which were acceptable but exhibited lower repeatability than those of SCP. Multivariate analyses showed that older age (p = 0.030) and greater axial length (p = 0.005) were significantly associated with lower repeatability of SCP FAZ area measurements. In addition, greater axial length (p = 0.044) was a significant factor for lower repeatability of deep capillary plexus FAZ area measurements. Conclusions: Manual measurement of FAZ area using OCTA exhibited relatively good repeatability for high myopia. Age and axial length affected repeatability and should be considered when analyzing FAZ areas in high myopia patients.

Background This study aim to evaluate neurovascular changes in both macula and optic nerve head (ONH) in early stages of diabetic retinopathy (DR) using combined quantitative optical coherence tomography angiography (OCTA) parameters.Methods We studied 194 right eyes from 64 type 2 diabetic patients without DR (DM), 64 diabetic patients with nonproliferative diabetic retinopathy (NPDR) and 64 age-matched healthy controls. OCTA parameters were analyzed using binary logistic regression models and receiver operating characteristic (ROC) curves.Results There was significant reduction of vessel density (VD) in both macula and ONH comparing DM patients with and without NPDR to controls. The area under the curve (AUC) of the ROC curve for NPDR versus control was 0.963 ( p <0.001), with sensitivity of 93.8% and specificity of 90.6%. The ROC curves for NPDR patients versus NoDR patients (including DM and control groups) exhibited an AUC of 0.923 ( p <0.001), with sensitivity of 90.6% and specificity of 82.8%. The thickness of the retinal nerve fiber layer (RNFL) in both macula and ONH was not significantly different among these three groups.Conclusions VD in both macula and ONH was simultaneously decreased prior to RNFL thinning in DM patients through the course proceeding from preclinical DR to NPDR. Combined analysis of macula and ONH parameters was an comprehensive and accurate OCTA metric to distinguish NPDR patients from healthy controls and DM patients without DR.
