Validity and repeatability of three in-shoe pressure measurement systems

In-shoe pressure measurement devices are used in research and clinic to quantify plantar foot pressures. Various devices are available, differing in size, sensor number and type; therefore accuracy and repeatability. Three devices (Medilogic, Tekscan and Pedar) were examined in a 2 day x 3 trial design, quantifying insole response to regional and whole insole loading. The whole insole protocol applied an even pressure (50-600 kPa) to the insole surface for 0-30 seconds in the Novel TruBlue TM device. The regional protocol utilised cylinders with contact surfaces of 3.14 and 15.9cm 2 to apply pressures of 50 and 200 kPa. The validity (% difference and Root Mean Square Error: RMSE) and repeatability (Intra-Class Correlation Coefficient: ICC) of the applied pressures (whole insole) and contact area (regional) were outcome variables. Validity of the Pedar system was highest (RMSE 2.6 kPa; difference 3.9%), with the Medilogic (RMSE 27.0 kPa; difference 13.4%) and Tekscan (RMSE 27.0 kPa; difference 5.9%) systems displaying reduced validity. The average and peak pressures demonstrated high between-day repeatability for all three systems and each insole size (ICC≥0.859). The regional protocol contact area % difference ranged from -97 to +249%, but the ICC demonstrated medium to high between-day repeatability (ICC≥0.797). Due to the varying responses of the systems, the choice of an appropriate pressure measurement device must be based on the loading characteristics and the outcome variables sought. Medilogic and Tekscan were most effective between 200-300 kPa; Pedar performed well across all pressures. Contact area was less precise, but relatively repeatable for all systems.3

ABSTRACT. This study aimed at quantitatively evaluating the effectiveness of sheepskin sitting pad (SSP) in pressure relief in the wheelchair. Eighteen students were recruited from the university and those subjects with Body Mass Index (BMI) over 24.5 and with a history of back pain were excluded. Participants were guided to sit in a wheelchair, where contact pressure with and without SSP was measured by sitting pad pressure measurement. Four masks model was developed (total region, 2 left, right and central buttock) and under each region peak pressure (mmHg) (PP), mean pressure (MP) and contact area (cm ) (CA) were calculated. Wilcoxon Signed Ranks and reliability analysis were used for statistical analysis with a significant level of 0.05. Our results show that, with the assistance of SSP, CA at each area was improved, but those findings were not significantly recorded; further, mean MP was significantly reduced by 15.8 mmHg and the decrease of PP was less significant than that of MP. Most of our outcomes were approved to be reliable and repeatable. Overall, sheepskin sitting pad could significantly increase the contact area and reduce the intensity of pressure concentration; further, most of our outcomes were reliable and repeatable. Hence, sheepskin sitting pad is a useful pressure relieving product. S-au selectat optsprezece studenþi voluntari de la universitate, iar acei subiecþi cu indice de masã corporalã (IMC) de peste 24,5 ºi cu antecedente privind afecþiuni ale spatelui au fost excluºi. Participanþilor li s-a cerut sã se aºeze într-un scaun cu rotile, determinându-se presiunea de contact cu ºi fãrã perna din blanã ovinã prin mãsurarea presiunii exercitate asupra pernei pentru ºezut. S-a dezvoltat un model cu patru zone (regiune totalã, fesa stângã, fesa dreaptã ºi zona 2 centralã) ºi sub fiecare regiune s-au calculat presiunea maximã (mmHg) (PP), presiunea medie (MP) ºi zona de contact (cm ) (CA). S-au utilizat testul Wilcoxon Signed Ranks ºi testul de fiabilitate pentru analiza statisticã, cu un nivel de semnificaþie de 0,05. Rezultatele noastre aratã cã, la utilizarea pernei din blanã ovinã, s-a îmbunãtãþit CA pentru fiecare zonã, dar aceste constatãri nu au fost semnificative; în plus, MP a fost redusã semnificativ cu 15,8 mmHg, iar scãderea PP a fost mai puþin semnificativã decât cea a MP. Abstract. From a subject group of pes cavus, the purpose of this study was to evaluate the biomechanical characteristics of lower limbs, based on plantar foot pressure and electromyography (EMG) activities, by the effects on two kind of custommade insoles. Ten individuals among thirty females with a clinical diagnosis of idiopathic pes cavus (mean age (SD): 22.3 (0.08) years) were selected for the study. The plantar foot pressure data and EMG activities of four lower limb muscles were collected, when subjects walked on a treadmill, under three different experimental conditions. The plantar foot pressure data was analyzed, after the bilateral foot was divided into three areas of masks and into four sections of stance phase, to compare plantar foot pressure. The EMG activities were analyzed for integrated EMG (IEMG) value. The results show that plantar foot pressure concentrated in particular parts is decreased by custom-made insoles. In the case of EMG, all the muscle activities decreased significantly. The custom-made insoles dispersed pressure concentrated by the higher medial longitudinal arch and improved the efficient use of muscles. In particular, the extension structure in the forefoot of custom-made insoles was more efficient for pes cavus. Therefore, it could help patients to walk, by offering support to prevent the disease of pes cavus deformity, and to relieve the burden and fatigue in the lower limbs on gait.
