Rehabilitative ultrasound imaging (RUSI) technique seems to be a valid and reliable tool for diagnosis and treatment in physiotherapy and has been widely studied in the lumbopelvic region the last three decades. The aims for this utility in clinical settings must be review through a systematic review, meta-analysis and meta-regression. A systematic review was designed following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines with PROSPERO registration and per review in all phases of the process using COVIDENCE, analysis of risk of bias and meta-analysis using REVMAN, and meta-regression calculation using STATA. Database screening provided 6544 references, out of which 321 reported narrative synthesis, and 21 reported quantitative synthesis, while only 7 of them provided comparable data to meta-analyze the variables pain and muscle thickness. In most cases, the forest plots showed considerable I2 heterogeneity indexes for multifidus muscle thickness (I2 = 95%), low back pain (I2 = 92%) and abdominal pain (I2 = 95%), not important for transversus abdominis muscle thickness (I2 = 22%), significant heterogenity (I2 = 69%) depending on the subgroup and not important internal oblique muscle thickness (I2 = 0%) and external oblique muscle thickness (I2 = 0%). Meta-regression did not provide significant data for the correlations between the variables analyzed and the intervention, age, and BMI (Body Mass Index). This review reveals that RUSI could contribute to a high reliability of the measurements in the lumbopelvic region with validity and reliability for the assessments, as well as showing promising results for diagnosis and intervention assessment in physiotherapy compared to the traditional model, allowing for future lines of research in this area.

The introduction of suburethral sling was a breakthrough in the treatment of stress urinary incontinence in women. The method is highly effective. However, the mechanism of action of a sling and the reasons for surgical failures are not fully understood. Aim: The aim of the study was to assess the impact of sling-pubic symphysis distance on eliminating the symptoms of stress urinary incontinence and urethral funneling. Materials and methods: The analysis included 106 patients who reported 3 to 6 months after sling placement for a follow-up visit encompassing clinical examination and standard ultrasonography. We evaluated the position of sling in relation to the pubic symphysis, urethral length, as well as urethral funneling length and width. Results: Cure criteria were met by 91 patients. Elimination of urethral funneling was achieved in 76.9% (n = 70) of cured patients. Urethral funneling was still present, yet shorter by a mean of 10.2 mm (p = 0.02) than before surgery in the remaining cured patients. There was a 32.5% reduction in the mean relative length of urethral funneling (p = 0.002). No significant differences were found in the pre- and postoperative funneling width. The tape-pubic symphysis distance was lower in cured women: 23.2 mm vs. 26.1 mm in failed women (p = 0.04). Similar observations were made for cured patients with persistent urethral funneling vs. failed patients (22.47 mm vs. 26.0 mm, p = 0.027). There were no differences between cured patients without urethral funneling and cured patients with persistent postoperative funneling (23.5 mm; 22.5 mm; p = 0.417). Conclusions: Tape position in relation to the pubic symphysis is important for the elimination of stress urinary incontinence. Sling location closer to the pubic symphysis reduces the length of urethral funneling, but has no effects on its width in cured patients with persistent postoperative funneling.

ZusammenfassungDie Urogynäkologie ist ein hochspezialisierter interdisziplinärer Fachbereich vom Charakter einer Subspezialisierung. Sie verfolgt das Ziel der Untersuchung und nachhaltigen ganzheitlichen Behandlung funktioneller, statischer, endokriner und sexueller Störungen bzw. Erkrankungen im Bereich des Urogenitalsystems und des kleinen Beckens. Der Arbeitskreis „Urologische Funktionsdiagnostik und Urologie der Frau“ der Akademie der Deutschen Urologen wurde 1977 gegründet und fokussiert die Urogynäkologie auf die Aktualisierung und Fortentwicklung der Diagnostik und klinischen Therapieverfahren, auf die Weiterbildung sowie auf die Mitgestaltung der urogynäkologischen Leitlinien. Harninkontinenz und Beckenbodensenkungen nehmen demografisch bedingt zu. Steigende soziokulturelle Ansprüche der Frauen erfordern effektivere Behandlungen mit erfolgssicheren und komplikationsarmen Methoden. Meilensteine in der Urogynäkologie waren die Einführung und Weiterentwicklung minimalinvasiver Operationsverfahren wie unter anderem TVT („tension-free vaginal tape“) -Systeme, laparoskopische und roboterassistierte Techniken, die Entwicklung eines Komplikationsmanagements, gezieltere medikamentöse Therapien sowie die Möglichkeiten der modernen Funktionsdiagnostik mit Beckenbodensonografie und MRT-Untersuchungen. Klassische Operationstechniken wie Kolposuspensionen oder Kolposakropexien und vaginale Plastiken ohne Fremdmaterial haben weiterhin einen hohen Stellenwert. In speziellen Fällen sowie Rezidivsenkungen haben sich netzbasierte pelvine Plastiken den konventionellen Verfahren wegen guter Langzeiterfolge und nur geringen Rezidivraten als überlegen erwiesen, sofern eine anatomisch korrekte Operationstechnik, kritische Auswahl textiler Implantate sowie eine angemessene Aufklärung erfolgt. Instrumentelle Nahttechniken und Operationsroboter sind aufgrund der beengten Zugänge und Aktionsräume vorteilhaft. Bei apikalen direkten Fixationen ist die therapeutisch relevante Zielgröße der Elevationswinkel der Scheide. Bewährte anatomische Fixationspunkte sind vorzuziehen.Die Aus- und Weiterbildung auf dem Fachgebiet der Urogynäkologie und die Harmonisierung mit europäischen Vorgaben (European Board & College of Obstetrics and Gynaecology) sollten umgesetzt werden. Fähigkeiten und Fertigkeiten zu hochspezialisierten netzbasierten Eingriffen und effektiven Techniken zum Komplikationsmanagement sollten genauso interdisziplinär vermittelt werden, wie einfach zu erlernende und zu nutzende diagnostische Verfahren, besonders aber die Beckenbodensonografie.Nur eine Fächerübergreifende urogynäkologische Subspezialisierung kann die Entwicklung erforderlicher Fachkompetenzen garantieren. Interdisziplinäre Weiterbildungen sollten durch die beteiligten Fachgesellschaften (Urologie, Chirurgie und Gynäkologie) inhaltlich festgelegt und deren Umsetzung vereinbart werden.
