Repeatability and agreement of biometric measurements using spectral domain anterior segment optical coherence tomography and Scheimpflug tomography in keratoconus

Abstract: Purpose To compare the repeatability and agreement in biometric measurements using Spectral Domain Anterior Segment OCT (AS-OCT, REVO-NX, Optopol) and Scheimpflug tomography (Pentacam-AXL, Oculus) in keratoconus. Methods Prospective case series at a university hospital tertiary center. Axial length (AL), anterior chamber depth (ACD), central corneal thickness (CCT), and thinnest corneal thickness (TCT) were measured using both devices in patients with keratoconus. Three groups were analyzed: eyes with no pri… Show more

“…22 Furthermore, the TCT of patients with KC1, 2, and 3 keratoconus measured by Casia-2 were 534.7 ± 39.9 µm, 578.1 ± 37.4 µm, and 450.7 ± 37.3 µm, respectively. 23 These results are comparable to those of our study. In contrast, the CCT measured by RTVue reported earlier in mild to moderate non-progressive keratoconus were 457.3 ± 38.9 µm and 431 ± 39.7 µm, lower than the CCTs measured by RTVue in our study, probably because the former study only included cases with stages 2-3 based on the Amsler-Krumeich classi cation criteria.…”

“…However, most previous studies on patients with keratoconus reported that FD-OCT tend to underestimate the CCT compared with Pentacam. 20,23 Patient selection may explain the discrepancy between those results and ours; previous studies included keratoconus patients with a wide range of stages, including forme fruste or KC3-4 keratoconus. Also, corneal thickness is obtained by measuring the radial distance between two concentric spheres.…”

“…We used Bland–Altman plots to illustrate the agreements among the three devices for CCT measurements; an acceptable agreement was noted for all (±15 μm < LOA < ±30 μm). However, most previous studies on patients with keratoconus have reported that compared to Pentacam, FD-OCT tends to underestimate CCT [ 17 , 19 ]. Patient selection may explain the discrepancy between these previous results and our present findings; previous studies included patients with keratoconus at a wide range of stages, including forme fruste or KC3–4 keratoconus.…”

“…Li et al, studied (CCT) and (TCT) in KC eyes using both Pentacam and AS-OCT measurements. Although both machines demonstrated good repeatability, there was poor agreement between different groups [ 20 ]. This contrasts with our work where a good agreement for CCT & TCT between both machine readings was demonstrated in KC patients.…”
