Repair of Type A Dissection with Mitral Regurgitation Using Total Arch Replacement with Mitral Valve Surgery

Abstract: Repair of type A dissection with MR can be performed in a single operation. Combined TAR and mitral valve surgery is a much bigger and more complex operation associated with high morbidity.

“…In this case, the dilated aortic annulus made the exposure of the MV apparatus satisfactory [7]. Our previous reports also confirmed that a transverse aortotomy MV operation was a feasible surgical approach to treat patients with aortic root aneurysm or type A dissection combined with mitral valve lesions [5,19]. To date, there is no comparative study of MV surgery via a transverse aortotomy or the transatrial septum.…”

“…Currently, aortic arch replacement surgery is typically performed under moderate or deep hypothermia and circulatory arrest because the open anastomosis at the distal end of the arch needs to be performed without blood supply to the spinal cord and abdominal organs [13]. To improve the body’s tolerance to ischaemia, continuous cooling is necessary.…”

“…Patients with Marfan syndrome, arch rupture, or atypical origin of the arch branches were not deemed suitable candidates for the use of this specialty device. Several of the patients who underwent a full sternotomy possibly required a coronary bypass graft or a mitral or tricuspid valve repair, which are known to be associated with higher rates of morbidity and mortality 6 ; however, these data were not presented in detail. Also important to note is that the average patient age was 55 years and that the body mass index in only a few patients was greater than 29 kg/m 2 .…”
