Repair of fistulas-in-ano using autologous fibrin tissue adhesive

Abstract: Our initial results are optimistic and require further support through longer follow-up data. Fibrin glue treatment of anorectal fistulas offers a unique mode of management that is safe, simple, and easy for the surgeon to perform. By using autologous fibrin tissue adhesive the patient avoids the risk of anal incontinence and the discomfort of prolonged wound healing which may be associated with fistulotomy.

“…They reported an overall closure rate of 74% (17/ 23); however, 5 patients (22%) required 2 or 3 attempts at sealant closure. Results of our original study, 6 which used ethanol-based autologous fibrin tissue adhesive (AFTA-E), and our present study are similar to those of previous reported studies. Our surgical protocol differs, however, from other studies in that we do not treat our patients preoperatively with bowel preparations or antibiotics, and our patients are not hospitalized and are sent home postoperatively on a general diet.…”

“…In one study, 26 patients with fistulae in ano were treated using autologous fibrin tissue adhesive that used a combination of ethanol and freezing to precipitate fibrinogen (AFTA-E). 6 Twentyone (81%) of the 26 patients had success- ful closure of their fistula after a 4-month follow-up. Two of the 5 failures were reinjected a second time and 1 closed, giving an overall successful closure rate of 85% (22 of 26 patients).…”

“…v. 46 -no. 4 Com relação aos 27 pacientes nos quais não houve cicatrização, em 9 pacientes (33,3%) a falha do tratamento foi diagnosticado nos primeiros 30 dias após a cirurgia, em 13 (48,8%) entre 1 e 3 meses, em 3 (11,1%) entre 3 e 6 meses e em 2 pacientes (7,4%) entre 6 e 9 meses após a cirurgia. Não foram observadas complicações relacionadas ao tratamento.…”

“…In the largest published series to date, CINTRON et al (4) reported a healing rate for cryptoglandular fistulas of 63%, compared with 35% for fistulas of other etiologies, with a mean follow-up of 1 year. Although no study has been able to demonstrate a statistically significant difference in healing rates according to fistula etiology (20) , it is generally agreed that anal fistulas associated with Crohn's diseases or HIV are usually more difficult to heal.…”

“…These techniques have less risk of anal incontinence, despite some recurrences. Several studies on fibrin glue treatment of anal fistula have been published since 1999, but, the healing rate after debridement and fibrin glue injection is 14%-60% [22][23][24][25][26] . Use of anal fistula plug for complex fistula-in-ano repair [27,28] has a reported success rate of 83%.…”
