Reoperation for recurrent peptic ulceration

Abstract: Thirty-eight patients undergoing reoperation for recurrent peptic ulceration have been reviewed after at least 3 years. All were considered to be in Visick grade IV before reoperation. There were no operative deaths. Thirty-three of the patients ended with both a vagotomy and antral resection. After reoperation 17 patients (45 per cent) remained in Visick grades III and IV. Eight patients have had further operations and, after 47 procedures in 38 patients, 14 (37 per cent) were still in Visick grades III and I… Show more

“…Miranda et al [28] state that in 50% of their cases with an antiperistal tic jejunal interposition, the dumping syndrome was just exchanged for EGR. Several authors claim that recon struction of pylorus is a procedure suitable for treating gastric incontinence to some extent, but has little to offer in cases with EGR [7,13,25], Muscroft et al [29] report an extremely high morbid ity of 47% after antrectomy with Billroth I gastroduodenostomy for recurrent ulceration and conclude that another type of gastroenteroanastomosis such as Rouxen-Y must be considered; a conclusion also supported by Cooper and Bell [10], who reported a very low morbidity after antrectomy with Roux-en-Y reconstruction for re current ulceration.…”

“…Few patients in the present series remained failures, in the real sense, since the majority of the patients with recurrence had this recurrence demonstrated only once and fared very well both before and after that seemingly isolated event. This apparently benign course of many recurrences after HSV has been mentioned by several authors [1][2][3][4] and is in contrast to that of roe after other operations which are often diiticult to treat [5,6].…”

“…If a recurrent ulcer had been demonstrated during that period, the grading was Visick 4, otherwise a grade was given according to the patient's condition. Patients who required reoperation (3 for recurrent ulceration and 2 for stenosis) could applied by some authors [6,[14][15][16]. In the present study, no correction for experience was applied and no patients were excluded.…”
